目的:观察活动期类风湿关节炎( RA)患者血脂特征及其临床意义。方法选取活动期RA患者(活动组)40例、非活动期RA患者(非活动组)36例与健康对照组30例,同时对受试者血清总胆固醇( TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)及C-反应蛋白(CRP)、类风湿因子(RF)、红细胞沉降率(ESR)进行检测。结果与健康对照组比较,2组RA患者HDL-C、ApoA1显著降低( P <0殮.01),ESR、CRP、RF升高( P <0.01),且活动组较非活动组变化更明显( P均<0.01),而TC、TG、LDL-C、ApoB 3组间比较差异无统计学意义( P﹥0.05);HDL-C、ApoA1与CRP、ESR、RF均呈显著负相关(r =-0.634、r =-0.901、r =-0.485, r =-0.625、r =-0.520、r =-0.236, P <0.05或P <0.01);TC、TG、LDL-C、ApoB与疾病活动性无明显相关( P﹥0.05)。结论活动期RA患者体内HDL-C、ApoA1显著降低,这种变化与RA病情活动密切相关。

목적:관찰활동기류풍습관절염( RA)환자혈지특정급기림상의의。방법선취활동기RA환자(활동조)40례、비활동기RA환자(비활동조)36례여건강대조조30례,동시대수시자혈청총담고순( TC)、삼선감유(TG)、고밀도지단백담고순(HDL-C)、저밀도지단백담고순(LDL-C)、재지단백A1(ApoA1)、재지단백B(ApoB)급C-반응단백(CRP)、류풍습인자(RF)、홍세포침강솔(ESR)진행검측。결과여건강대조조비교,2조RA환자HDL-C、ApoA1현저강저( P <0렴.01),ESR、CRP、RF승고( P <0.01),차활동조교비활동조변화경명현( P균<0.01),이TC、TG、LDL-C、ApoB 3조간비교차이무통계학의의( P﹥0.05);HDL-C、ApoA1여CRP、ESR、RF균정현저부상관(r =-0.634、r =-0.901、r =-0.485, r =-0.625、r =-0.520、r =-0.236, P <0.05혹P <0.01);TC、TG、LDL-C、ApoB여질병활동성무명현상관( P﹥0.05)。결론활동기RA환자체내HDL-C、ApoA1현저강저,저충변화여RA병정활동밀절상관。
Objective To observe the characteristics of serum lipids in activity rheumatoid arthritis ( RA) and inves-tigate its clinical significance.Methods Select active RA patients (active group) 40 cases, non-active RA patients (inactive group) 36 cases, and healthy control group of 30 patients, while the subjects serum total cholesterol ( TC), triglyceride (TG), high-density lipoprotein cholesterol ( HDL-C), low density lipoprotein cholesterol ( LDL-C), apolipoprotein A1 (ApoA1), apolipoprotein B (ApoB) and C reactive protein (CRP), rheumatoid factor (RF), erythrocyte sedimentation rate (ESR) were detected.Results Compared with the healthy control group , two groups of RA patients'HDL-C, ApoA1 de-creased significantly ( P <0.01), ESR,CRP,RF increased significantly( P <0.01),and the active group changed more ob-vious than the inactivity group ( P <0.01) , and TC, TG, LDL-C, ApoB in 3 groups showed no statistically significant ( P >0.05);HDL-C, ApoA1 presented significant negative correlation with CRP , ESR, RF ( r =-0.634, r =-0.901, r =-0.485, r =-0.625, r =-0.520, r =-0.236, P <0.05 or P <0.01); TC, TG, LDL-C, ApoB and disease activity was not significant correlated ( P >0.05).Conclusion In the active phase of RA, serum HDL-C and ApoA1 levels decrease significantly, this change is closely related with the disease activity .
