目的:探讨营养支持对ⅡB~ⅢA 期非小细胞肺癌(NSCLC)患者手术前新辅助化疗后肺功能的影响。方法收集50例术前ⅡB~ⅢA 期 NSCLC 患者,分为对照组和试验组各25例,对照组:普通饮食,行含紫杉类+铂类方案化疗;试验组:在对照组基础上增加化疗期间的营养支持治疗;对化疗前、后的肺功能指标第1秒用力呼气量(FEV1)、最大分钟通气量(MVV)及弥散功能肺一氧化碳弥散量(DLCO)进行检测。结果两组化疗前肺功能指标差异均无统计学意义(P >0.05);化疗后 DLCO 较化疗前有显著下降,差异有统计学意义(P <0.05)。试验组化疗后 FEV1、MVV 较化疗前及对照组化疗后有提高,差异有统计学意义(P <0.05)。结论术前化疗对肺弥散功能产生一定不良影响,有一定的肺毒性,营养支持能在一定程度上改善ⅡB~ⅢA 期非小细胞肺癌患者术前新辅助化疗后肺通气功能,但对肺弥散功能无显著影响。

목적:탐토영양지지대ⅡB~ⅢA 기비소세포폐암(NSCLC)환자수술전신보조화료후폐공능적영향。방법수집50례술전ⅡB~ⅢA 기 NSCLC 환자,분위대조조화시험조각25례,대조조:보통음식,행함자삼류+박류방안화료;시험조:재대조조기출상증가화료기간적영양지지치료;대화료전、후적폐공능지표제1초용력호기량(FEV1)、최대분종통기량(MVV)급미산공능폐일양화탄미산량(DLCO)진행검측。결과량조화료전폐공능지표차이균무통계학의의(P >0.05);화료후 DLCO 교화료전유현저하강,차이유통계학의의(P <0.05)。시험조화료후 FEV1、MVV 교화료전급대조조화료후유제고,차이유통계학의의(P <0.05)。결론술전화료대폐미산공능산생일정불량영향,유일정적폐독성,영양지지능재일정정도상개선ⅡB~ⅢA 기비소세포폐암환자술전신보조화료후폐통기공능,단대폐미산공능무현저영향。
Objective To study the influence of lung function of pre-operation patients with stage ⅡB-ⅢA non-small cell lung cancer after neoadjuvant chemotherapy by nutrition support.Methods Fifty pre-operation patients with stage ⅡB-ⅢA non-small cell lung cancer were randomly divided into two groups:the control group (25 cases):normal diet,containing paclitaxel + platinum-based chemotherapy;experimental group (25 cases):increased the nutritional support during the chemotherapy treatment on the basis of the control group;the lung function indexes were tested such as FEV1,MVV,DLCO before and after chemotherapy.Results The lung function indexes had no significant difference before chemotherapy between the control group and the experimental group (P > 0.05 );DLCO dropped significantly after the chemotherapy,the difference was statistically significant (P < 0.05 ).FEV1,MVV in the experimental group relatively rose compared with pre-chemotherapy and the control group after chemotherapy(P <0.05 ). Conclusion Preoperative chemotherapy has some influence on pulmonary diffusion function,there is certain lung toxicity.Nutrition support can improve pulmonary ventilation function of patients with stage ⅡB-ⅢA non-small cell lung cancer after preoperative neoadjuvant chemotherapy in some extent,but it has no significant effect on pulmonary diffusion function.
