目的规范临床抗菌药物合理使用。方法西北地区某三级甲等医院质量控制部门分析确定医院应用抗菌药物存在的主要问题，建立了医院抗菌药物合理使用管理体系。研究选取医院2012年10月-11月300例住院病例作为干预组，选取2012年7-8月300例住院病例作为对照组，进行抗菌药物是否合理使用的对照研究。利用病历检查和从医院信息系统提取数据的方法，对住院患者抗菌药物合理使用指标和围手术期预防用药指标进行分析。结果干预前抗菌药物使用中存在的主要问题，如抗菌药物品种选择不当，给药时机不当，用药疗程过长和联合用药率高等，在干预后均有显著改善。结论在医院建立抗菌药物临床应用管理体系有助于促进临床一线安全、有效、经济、合理地使用抗菌药物，值得推广借鉴。

Objectives:Standardize the use of clinical antibiotics. Methods:The quality control department of a Three-A hospital in northwest China analyzed the major problems existing in its use of antibiotics, and established a management system. The study selected 300 inpatient cases in the hospital during the October and November of 2012 as the intervention group, while 300 during the July and August of the same year as the control group, and conducted control study on whether antibiotics were reasonably used. By way of case history examination and data search in the hospital's information system, indices including inpatient antibiotics usage and peri-operative prevention usage were analyzed. Results:Major problems existed before intervention, such as inappropriate antibiotics selection, inappropriate administration time, over-length of usage, and high rate of combination therapy, were significantly ameliorated. Conclusions:Establishing the management system for the clinical use of antibiotics is helpful for promoting its safe, effective, economical, and reasonable use. It is worth popularizing.
