目的:探讨甲状腺功能亢进症患者糖耐量与胰岛素抵抗的变化与发病机制。方法:回顾性分析本院2012年1月至2013年6月收治的甲状腺功能亢进患者40例(实验组)与健康体检者40例(对照组)。对两组患者空腹血糖(F P G)、餐后2h血糖(2h P G)和胰岛素量进行测定,记录其胰岛素分泌指数(HOMA-IS)、胰岛素敏感指数(ISI)、胰岛素抵抗指数(HOMA-IR),并对两组测量数据进行比较分析。结果:实验组患者FPG、2hPG、空腹胰岛素水平(FINS)、2h胰岛素水平(2hINS)、HOMA-IR均较对照组明显升高(P<0.05); HOMA-I S较对照组略有降低(P<0.05),而I S I则较对照组明显降低(P<0.01)。结论:甲状腺功能亢进症患者大多存在糖耐量降低与胰岛素抵抗。提示在治疗甲状腺功能亢进的同时,要注意对各种血清生化指标进行监测,及时纠正高血糖症和高胰岛素血症,避免合并糖尿病发生的风险。

목적:탐토갑상선공능항진증환자당내량여이도소저항적변화여발병궤제。방법:회고성분석본원2012년1월지2013년6월수치적갑상선공능항진환자40례(실험조)여건강체검자40례(대조조)。대량조환자공복혈당(F P G)、찬후2h혈당(2h P G)화이도소량진행측정,기록기이도소분비지수(HOMA-IS)、이도소민감지수(ISI)、이도소저항지수(HOMA-IR),병대량조측량수거진행비교분석。결과:실험조환자FPG、2hPG、공복이도소수평(FINS)、2h이도소수평(2hINS)、HOMA-IR균교대조조명현승고(P<0.05); HOMA-I S교대조조략유강저(P<0.05),이I S I칙교대조조명현강저(P<0.01)。결론:갑상선공능항진증환자대다존재당내량강저여이도소저항。제시재치료갑상선공능항진적동시,요주의대각충혈청생화지표진행감측,급시규정고혈당증화고이도소혈증,피면합병당뇨병발생적풍험。
Objective: On patients with hyperthyroidism pathogenesis with the change of glucose tolerance and insulin resistance.Methods: Retrospective analysis from January 2012 to June 2013 were between 40 patients with thyroid function hyperfunction (experimental group) and 40 patients with healthy group (control group). On two groups of patients with fasting plasma glucose (FPG), 2 h postprandial blood glucose (2 hPG) and insulin were determined, record its insulin secretion index (HOMA - IS), insulin sensitive index (ISI), insulin resistance index (HOMA - IR), and a comparative analysis of two groups of measuring data.Results: FPG, 2 hPG experimental group patients was signiifcantly higher (P < 0.05); Group fasting insulin (FINS), insulin level 2 hours (2 hins) higher than control group obviously (P < 0.05); Insulin resistance index (HOMA IR) were signiifcantly higher than the control group (P < 0.05); Insulin secretion index (HOMA - IS) relatively slightly lower than control (P< 0.05), and insulin sensitivity index (ISI) were signiifcantly lower than the control group (P < 0.01). Conclusion: Hyperthyroidism patients usually exist to reduce glucose tolerance and insulin resistance. In the treatment of thyroid function hyperfunction at the same time, to pay attention to all kinds of the serum biochemical indicators monitoring, timely correcting, hyperglycemia and hyperinsulinemia is avoid the risk of diabetes mellitus happen.
