目的：评价慢性病自我管理项目（CDSMP)在妇科恶性肿瘤经外周静脉置入中心静脉导管(PICC)化疗患者中的实施效果。方法：将182例PICC置管肿瘤患者随机分为对照组和干预组各91例，对干预组PICC化疗患者实施CDSMP，对照组化疗患者PICC管理由护士进行，比较两组患者化疗结束后PICC拔管前自我效能、疾病自我管理能力、健康状况、PICC自我管理能力及并发症发生率等方面的差异。结果：与对照组比较，干预组的自我效能明显提高，在健康状况的5个方面明显优于对照组(P<0.05)，PICC导管自我管理能力明显提高，相关导管并发症发生率明显低于对照组（P<0.05）。结论：CDSMP可改善妇科恶性肿瘤PICC化疗患者的自我效能、提高自我管理行为，并直接影响PICC留置期间患者导管安全。

목적：평개만성병자아관리항목（CDSMP)재부과악성종류경외주정맥치입중심정맥도관(PICC)화료환자중적실시효과。방법：장182례PICC치관종류환자수궤분위대조조화간예조각91례，대간예조PICC화료환자실시CDSMP，대조조화료환자PICC관이유호사진행，비교량조환자화료결속후PICC발관전자아효능、질병자아관리능력、건강상황、PICC자아관리능력급병발증발생솔등방면적차이。결과：여대조조비교，간예조적자아효능명현제고，재건강상황적5개방면명현우우대조조(P<0.05)，PICC도관자아관리능력명현제고，상관도관병발증발생솔명현저우대조조（P<0.05）。결론：CDSMP가개선부과악성종류PICC화료환자적자아효능、제고자아관리행위，병직접영향PICC류치기간환자도관안전。
Objective:To evaluate the implementation effects of Chronic Disease Self-Management Program (CDSMP) among the patients with gynecological malignancies and peripherally inserted central catheter (PICC) chemotherapy. Methods:Totally 182 patients were randomly divided into control group (n=91) and intervention group (n=91). CDSMP was carried out in intervention group while conventional PICC catheter care was carried out in control group. The changes of self-efifcacy, disease self-management ability, health status, PICC self-management ability and complications after chemotherapy in the two groups were compared. Results:Comparing to the control group, self-efifcacy was improved in the intervention group. Five aspects of health status (self-evaluation of whole health, emotion control, energy index, fatigue and effect of PICC on life) of the patients in intervention group were better than those in control group (P<0.05). Self-management ability of PICC catheter in intervention group was improved signiifcantly. Incidences of complications including obstruction of catheter, mechanical phlebitis, drop of catheter and catheter infection in intervention group were signiifcantly lower than those in control group (P<0.05). Conclusion:CDSMP can improve self-efifcacy and self-management ability of patients with gynecological malignancies and affect the security of catheter during the period of PICC indwelling.
