目的：观察大剂量阿托伐他汀对急性脑梗死（ ACI）患者血脂、神经功能恢复情况及1年复发率的影响。方法120例发病72 h内入院的ACI患者随机分为2组，口服阿托伐他汀20 mg/d组（对照组）和40 mg/d组（治疗组）各60例，观察治疗前和治疗4 w后患者血脂水平，用美国国立卫生研究所脑卒中评分（ NIHSS）评定患者神经功能缺损程度，观察治疗1年后患者生活能力Barthel指数（ BI）、脑缺血事件复发率。结果治疗4 w后，两组血脂水平、神经功能缺损程度较治疗前明显好转（P＜0．01），但治疗组均明显优于对照组（P＜0．01）。1年后生活能力BI指数、复发率，两组均较治疗前明显改善（P＜0．01），治疗组改善明显优于对照组（P＜0．05）。结论大剂量阿托伐他汀治疗ACI，可更有效地降低血脂水平，促进神经功能恢复，提高患者的生活能力，并降低脑缺血事件复发率。
목적：관찰대제량아탁벌타정대급성뇌경사（ ACI）환자혈지、신경공능회복정황급1년복발솔적영향。방법120례발병72 h내입원적ACI환자수궤분위2조，구복아탁벌타정20 mg/d조（대조조）화40 mg/d조（치료조）각60례，관찰치료전화치료4 w후환자혈지수평，용미국국립위생연구소뇌졸중평분（ NIHSS）평정환자신경공능결손정도，관찰치료1년후환자생활능력Barthel지수（ BI）、뇌결혈사건복발솔。결과치료4 w후，량조혈지수평、신경공능결손정도교치료전명현호전（P＜0．01），단치료조균명현우우대조조（P＜0．01）。1년후생활능력BI지수、복발솔，량조균교치료전명현개선（P＜0．01），치료조개선명현우우대조조（P＜0．05）。결론대제량아탁벌타정치료ACI，가경유효지강저혈지수평，촉진신경공능회복，제고환자적생활능력，병강저뇌결혈사건복발솔。
Objective To observe the efficacy of high-dose atorvastatin in nervous functional rehabilitation in patients with acute cerebral infarction ( ACI) and 1-year recurrence rate .Methods 120 cases suffered from ACI in 72 h were randomly divided into 2 groups:conventional-dose atorvastatin treatment group(control group,20 mg/d)and high-dose atorvastatin treatment group(treatment group,40 mg/d),60 cases in each group.Before and after 4 weeks treatment,their blood lipid level was measured and the degree of neurological deficits was evaluated with the U .S.National Institutes of Health Stroke Score ( NIHSS ) .The Barthel index ( BI ) and cerebral ischemic event recurrence rate were observed after one year .Results The level of blood lipid and the degree of neurological deficits in both groups were significantly better after 4 weeks treatment than those before treatment (P<0.01);BI and the recurrence rate in both groups after one year were significantly better than those before treatment (P<0.01).The therapeutic effect was better in the treatment group than that in the control group (P<0.05).Conclusions High-dose atorvastatin has a good effect on ACI and can reduce 1-year recurrence rate .
