目的:探讨色素衍生上皮因子(PEDF)及血管内皮生长因子(VEGF)表达与肝外胆管癌的关系,并对胆管癌组织中两者的相关性进行初步研究。方法:收集中国医科大学附属盛京医院普通外科于2006年1月-2010年5月手术切除并经病理证实的肝外胆管癌标本70例。30例正常胆管组织取自胆囊结石患者行胆囊切除且无炎症状态的胆囊管组织。用免疫组织化学方法检测 PEDF 及 VEGF 的表达情况,统计学处理采用SPSS 19.0软件,组间关系采用单因素方差分析,相关关系分析采用 SPearman 等级相关分析法。结果:PEDF表达于细胞膜和细胞浆,以细胞膜和细胞浆出现棕黄色为阳性表达,其在胆管癌组织中有阴性或弱阳性表达,在正常胆管组织中有阳性表达。VEGF 表达于细胞浆,以细胞浆出现棕黄色或棕褐色颗粒为阳性表达,其在胆管癌组织中有阳性表达,在正常胆管组织中为阴性或弱阳性表达。在胆管癌组织中 PEDF 与 VEGF 的表达呈负相关。PEDF 和 VEGF 在胆管癌中的表达与有无淋巴结转移及肿瘤分期有关,而与肿瘤大小、分化程度无关。结论:PEDF 表达减少或 VEGF 表达增高与胆管癌的发生、发展及侵袭转移有关。PEDF 与 VEGF 在胆管癌的发生和发展中起到相反的作用。

목적:탐토색소연생상피인자(PEDF)급혈관내피생장인자(VEGF)표체여간외담관암적관계,병대담관암조직중량자적상관성진행초보연구。방법:수집중국의과대학부속성경의원보통외과우2006년1월-2010년5월수술절제병경병리증실적간외담관암표본70례。30례정상담관조직취자담낭결석환자행담낭절제차무염증상태적담낭관조직。용면역조직화학방법검측 PEDF 급 VEGF 적표체정황,통계학처리채용SPSS 19.0연건,조간관계채용단인소방차분석,상관관계분석채용 SPearman 등급상관분석법。결과:PEDF표체우세포막화세포장,이세포막화세포장출현종황색위양성표체,기재담관암조직중유음성혹약양성표체,재정상담관조직중유양성표체。VEGF 표체우세포장,이세포장출현종황색혹종갈색과립위양성표체,기재담관암조직중유양성표체,재정상담관조직중위음성혹약양성표체。재담관암조직중 PEDF 여 VEGF 적표체정부상관。PEDF 화 VEGF 재담관암중적표체여유무림파결전이급종류분기유관,이여종류대소、분화정도무관。결론:PEDF 표체감소혹 VEGF 표체증고여담관암적발생、발전급침습전이유관。PEDF 여 VEGF 재담관암적발생화발전중기도상반적작용。
Objective:The PEDF and VEGF exPression in extrahePatic bile duct carcinoma tissue and normal tis-sue were assessed by immunohistochemistry to study the correlation between the exPression of PEDF and VEGF. And we also concern with the relationshiP between their exPression and cholangiocarcinoma. Methods:Collect extrahePatic bile duct carcinoma tissue from 70 cholangiocarcinoma Patients normal tissue from 30 cholecystolithiasis Patients' bile duct that no iflammation as the samPles in the study. Patients were all PostoPerative and Pathological confirmed in the surgery dePartment of Shengjing eosPital of CMU from January,2006 to May,2010. The PEDF and VEGF exPressions were assessed by immunohistochemistry,and the data was analyzed by SPSS 19. 0. Results:The Positive signal of PEDF exPression was stained yellow and mainly located in the cell cytoPlasma and on the cell membrane. It mainly exPressed negative or weak Positive in carcinoma tissue and Positive or weak Positive in normal tissue. The Positive signal of VEGF exPression was stained yellow or brown and mainly located in the cell cytoPlasma and on the cell membrane. It mainly exPressed Positive in carcinoma,but negative or weak Positive in normal tissue. There was nega-tive correlation between PEDF and VEGF exPressions(r = - 0. 392,P ﹤ 0. 05). PEDF and VEGF exPression showed the correlation with lymPh node metastasis and neoPlasm staging,but with no correlation with tumor size or histological grades. Conclusion:Low exPression of PEDF has the correlation with occurrence,develoPment,invasion and metasta-sis of cholangiocarcinoma. eigh exPression of VEGF has the correlation with occurrence,develoPment,invasion and me-tastasis of cholangiocarcinoma.PEDF and VEGF have a converse function in the develoPment of cholangiocarcinoma.
