目的:对密度梯度离心法联合上游法制备精子的除菌效率进行评价。方法选择2013年12月-2014年1月在解放军202医院全军计划生育优生优育技术研究所进行辅助生殖技术治疗的男性志愿者100例,将精液培养无细菌感染者设为对照组(n=50),有细菌感染者设为实验组(n=50)。采用密度梯度离心法联合上游法进行精子制备,应用精液常规分析、细菌培养方法进行评估。结果实验组与精液原液比较,经密度梯度离心后精液上清液与沉淀物中细菌生长率比较均显著下降(P<0.05),经上游后精液上清液与沉淀物中细菌生长率下降更为明显(P<0.01),上清液培养无细菌生长。100份精液标本精子制备后前向运动与非前向运动精子之和百分比与制备前比较显著上升(P<0.05);实验组前向运动与非前向运动精子之和百分比与对照组比较差异无统计学意义(P>0.05)。结论采用密度梯度离心法联合上游法制备精子隔离细菌效果好,精子活动力显著提高。

Objective To evaluate the effectiveness of density gradient centrifugation technique combined with the swim-up method in sperm degerming preparation. Methods A total of 100 male volunteers received treatment of as-sisted reproductive technology during December 2013 and January 2014 in this study, and were divided into control group (n=50) without bacteria infection of the sperm and experimental group (n=50) with bacterial infections after the sper-moculture. The spermoculture was performed with density gradient centrifugation technique combined with swim-up meth-od. Seminal fluids were evaluated with conventional analysis and bacterial cultures methods. Results After preparation in experimental group, the bacterial rates in sperm supernate fluid and preciptitate were significantly lower after being cul-tured with density gradient centrifugation technique compared with that in stock solution (P<0. 05), and the rates were much more lower after culturing with the swim-up method (P<0. 01), and there was no bacterial in supernate fluid after culturing. The percentage of PR+NP ( prorsad+non-prorsad) movements in 100 sperm samples after preparation was significantly higher compare with that before preparation ( P <0. 05 ) , but the difference in the percentage of the two groups was not statistically significant ( P >0. 05 ) . Conclusion Density gradient centrifugation technique combined with the swim-up method in sperm degerming preparation may achieve better effect in bacteria isolation in semen, and im-prove the sperm motility.
