Electroacupuncture%Zhenqi Fuzheng Keli%Hepatic fibrosis%NF-κB%TGF-β1%CTGF
目的:探讨大鼠肝纤维化程度与自由基反应、核因子( NF)-κB 活化、转化生长因子( TGF)-β1mRNA 和结缔组织生长因子( CTGF) mRNA表达的关系及电针联合贞芪扶正颗粒的干预作用。方法雄性大鼠300只,随机分为正常对照组、模型组、电针组、贞芪扶正颗粒组、针药联合组。以二甲基亚硝胺( DMN)腹腔注射诱导大鼠肝纤维化模型,电针组造模同时给予电针足三里、关元、内关、三阴交、肝俞穴治疗,贞芪扶正颗粒组给予贞芪扶正颗粒灌胃,针药联合组则予上述电针和贞芪扶正颗粒治疗,共4w。造模4w后处死大鼠取肝脏组织标本,光镜观察肝脏组织的病理变化,并对肝脏组织标本进行病理评分;放射免疫法测定肝组织活性氧簇( ROS)、抗氧化能力( T-AOC)、超氧化物歧化酶( SOD)、谷胱甘肽过氧化物酶(GSH-Px)、过氧化氢酶(CAT)活性、脂质过氧化产物(LPO)含量及NF-κB活性,RT-PCR法检测肝组织中TGF-β1 mRNA及CTGF mRNA表达。结果与正常对照组比较,模型组大鼠的肝组织纤维化分期、ROS、LPO含量、NF-κB 活性、TGF-β1 mRNA 及 CTGF mRNA 表达显著增加( P<0.01),而T-AOC、SOD、GSH-Px、CAT活性明显下降( P<0.01);与模型组比较,各治疗组大鼠的肝组织纤维化积分、ROS、LPO含量、NF-κB活性、TGF-β1 mRNA及CTGF mRNA表达显著下降,而血清T-AOC、SOD、GSH-Px、CAT活性明显上升(P<0.01),联合治疗组上述指标优于其他治疗组(P<0.05)。 NF-κB活性与TGF-β1 mRNA及CTGF mRNA呈正相关(r=0.627,P<0.05;r=0.581,P<0.05)。结论自由基反应、NF-κB 活化、TGF-β1及 CTGF与肝纤维化的严重程度密切相关,对肝纤维化发生、发展有协同作用,电针和贞芪扶正颗粒均能显著抑制自由基反应,降低NF-κB活性,下调TGF-β1和CT-GF mRNA的表达,从而减轻肝组织损害程度,且联合用药效果更好。

목적:탐토대서간섬유화정도여자유기반응、핵인자( NF)-κB 활화、전화생장인자( TGF)-β1mRNA 화결체조직생장인자( CTGF) mRNA표체적관계급전침연합정기부정과립적간예작용。방법웅성대서300지,수궤분위정상대조조、모형조、전침조、정기부정과립조、침약연합조。이이갑기아초알( DMN)복강주사유도대서간섬유화모형,전침조조모동시급여전침족삼리、관원、내관、삼음교、간유혈치료,정기부정과립조급여정기부정과립관위,침약연합조칙여상술전침화정기부정과립치료,공4w。조모4w후처사대서취간장조직표본,광경관찰간장조직적병리변화,병대간장조직표본진행병리평분;방사면역법측정간조직활성양족( ROS)、항양화능력( T-AOC)、초양화물기화매( SOD)、곡광감태과양화물매(GSH-Px)、과양화경매(CAT)활성、지질과양화산물(LPO)함량급NF-κB활성,RT-PCR법검측간조직중TGF-β1 mRNA급CTGF mRNA표체。결과여정상대조조비교,모형조대서적간조직섬유화분기、ROS、LPO함량、NF-κB 활성、TGF-β1 mRNA 급 CTGF mRNA 표체현저증가( P<0.01),이T-AOC、SOD、GSH-Px、CAT활성명현하강( P<0.01);여모형조비교,각치료조대서적간조직섬유화적분、ROS、LPO함량、NF-κB활성、TGF-β1 mRNA급CTGF mRNA표체현저하강,이혈청T-AOC、SOD、GSH-Px、CAT활성명현상승(P<0.01),연합치료조상술지표우우기타치료조(P<0.05)。 NF-κB활성여TGF-β1 mRNA급CTGF mRNA정정상관(r=0.627,P<0.05;r=0.581,P<0.05)。결론자유기반응、NF-κB 활화、TGF-β1급 CTGF여간섬유화적엄중정도밀절상관,대간섬유화발생、발전유협동작용,전침화정기부정과립균능현저억제자유기반응,강저NF-κB활성,하조TGF-β1화CT-GF mRNA적표체,종이감경간조직손해정도,차연합용약효과경호。
Objective To explore the relationship between liver fibrosis degree and free radical reaction , NF-κB activation, TGF-β1 and CTGF mRNA expression in rats and the interventional effect of combining electroacupuncture and Zhenqi Fuzheng Keli on them .Meth-ods 300 male SD rats were randomly divided into five groups:normal control , model control , electroacupuncture , Zhenqi Fuzheng Keli and combination groups .The DMN was injected intraperitoneally for 4 weeks to induce liver fibrosis .At the same time of modeling , each rat in electroacupuncture group was treated with electroacupuncture of point Zusanli , Guanyuan, Neiguan, Sanyinjiao, Ganyu.While Zhenqi Fuzheng Keli was poured into gastric cavity in Zhenqi Fuzheng Keli group , and each rat was treated with above-mentioned puncture and Zhenqi Fuzheng Keli.The rats were killed after 4 weeks.The histomorphylogic structure of the liver tissue were observed under optical mi-croscope, and the damage degree of liver tissue was evaluated by a pathologic grading system .The levels of ROS, T-AOC, SOD, GSH-Px, CAT and LPO and NF-κB activity in liver tissue were determined by radioimmunoassay .The TGF-β1 and CTGF mRNA expression were measured with the methods of RT-PCR.Results Compared with normal control group , the fibrosis score, ROS and LPO levels, NF-κB ac-tivity, TGF-β1 and CTGF mRNA expression were increased obviously , the levels of T-AOC, SOD, GSH-Px and CAT were decreased obvi-ously in model group (P<0.01).Compared with model control group, the fibrosis score, ROS and LPO levels, NF-κB activity, TGF-β1 and CTGF mRNA expression were decreased obviously , the levels of T-AOC, SOD, GSH-Px and CAT were increased obviously in all treated groups (P<0.01), above-mentioned detecting indexes were better in combination group than those in other treated groups (P<0.05).There were obvious positive correlations among NF-κB activity, TGF-β1 and CTGF mRNA expression ( r1 =0.554, P<0.05; r2 =0.572, P<0.05).Conclusions Free radical reaction, activated NF-κB, TGF-β1 and CTGF are closely correlated to severity of hepatic fibrosis , there may be a synergetic interaction among free radical reaction , NF-κB, TGF-β1 and CTGF on the elevated pathogenesis of alcoholic hepat-ic fibrosis.Electroacupuncture and Zhenqi Fuzheng Keli can both inhibit free radical reaction , decrease NF-κB activity and downregulate TGF-β1 and CTGF mRNA expression obviously in rats with hepatic fibrosis to alleviate the severity of hepatic fibrosis and the effect of thera -peutic alliance is more better .
