目的:观察糖尿病视网膜病变(diabetic retinopathy,DR)激光光凝治疗的疗效,探讨最佳的光凝方式和时机。方法:对112例195眼(非增殖期60眼,增殖前期82眼,增殖期53眼)DR患者按病变程度分别行格子样视网膜光凝、局部视网膜光凝、次全视网膜光凝、全视网膜光凝及超全视网膜光凝治疗。术后随访1年以上,对比分析光凝前后视力、眼底及荧光血管造影结果的变化情况。结果:光凝治疗后173眼有效,总有效率为88.71%,其中非增殖期58眼有效(96.67%),增殖前期75眼有效(91.46%),增殖期43眼有效(81.13%)。治疗后55眼视力提高(28.20%),120眼视力不变(61.54%),20眼视力下降(10.25%)。结论:激光光凝术对治疗DR有显著疗效。应尽早发现、及时光凝以阻止病情发展,保护视功能。

목적:관찰당뇨병시망막병변(diabetic retinopathy,DR)격광광응치료적료효,탐토최가적광응방식화시궤。방법:대112례195안(비증식기60안,증식전기82안,증식기53안)DR환자안병변정도분별행격자양시망막광응、국부시망막광응、차전시망막광응、전시망막광응급초전시망막광응치료。술후수방1년이상,대비분석광응전후시력、안저급형광혈관조영결과적변화정황。결과:광응치료후173안유효,총유효솔위88.71%,기중비증식기58안유효(96.67%),증식전기75안유효(91.46%),증식기43안유효(81.13%)。치료후55안시력제고(28.20%),120안시력불변(61.54%),20안시력하강(10.25%)。결론:격광광응술대치료DR유현저료효。응진조발현、급시광응이조지병정발전,보호시공능。
Objective:To observe thetherapeutic effect of laser photocoagulation for diabetic retinopathy(DR),and to find the best way and the best chance of photocoagulation.Methods:According to the degree of the disease,grid photocoagulation, confluent local photocoagulation, mild-panretinal photocoagulation, panretinal photocoagulation and extra-panretinal photocoagulation were and separately performed on 195 eyes of 112 cases,among which there were 60 eyes for no-proliferative DR,82eyes for pre-proliferative DR and 53 eyes for proliferative DR.The postoperative fol ow-up was over 1 year.The visual acuity,fundus examinations and fundus fluorescein angiography(FFA) are analyzed comparatively.Results:The 173 eyes were effective among the 195 eyes after treatment;total effective rate was 88.81%.58 eyes were effective among the 60 eyes with the no-proliferative DR(effective rate 96.67%),75 eyes were effective among the 82 eyes with the pre-proliferative DR(effective rate 91.46%),43 eyes were effective among the 53 eyes with the proliferative DR(effective rate 81.13%).There were 55 eyes in which the final vision acuity had been increased (28.20%),120 eyes in which the final vision acuity had no change (61.54%) and 20 eyes in which the final vision acuity had been decreased (10.25%).Conclusions:Laser photocoagulation have notable curative effect for treating DR.The patients with DR should be found as early as possible and photocoagulation should be performed as soon as possible in order to prevent DR from the development and to protect visual function.
