目的:对老年患者下呼吸道感染病原菌分布及耐药性进行监测分析,为临床医生合理用药提供依据。方法:对医院2011年1月至2012年12月期间,老年下呼吸道感染患者的痰及下呼吸道分泌物中分离出来的1102例阳性标本,进行细菌鉴定和药敏试验。结果:1102株细菌中,革兰阴性杆菌776株,占70.4%,革兰阳性球菌210株,占19.1%;细菌学培养阳性前6位的为:铜绿假单胞菌、肺炎克雷伯菌、鲍曼不动杆菌、大肠埃希菌、金黄色葡萄球菌、真菌。革兰阴性杆菌对亚胺培南、头孢哌酮/舒巴坦、哌拉西林/他唑巴坦敏感性较高,均>70%,金黄色葡萄球菌对万古霉素、利奈唑胺敏感性较高,均为100%。结论:老年患者下呼吸道感染病原菌以革兰阴性杆菌为主,但对多种抗菌药物产生耐药,因此,临床应重视下呼吸道感染患者的标本送检率,根据药敏结果合理选用抗菌药物,使老年下呼吸道感染患者早诊断早治愈。

목적:대노년환자하호흡도감염병원균분포급내약성진행감측분석,위림상의생합리용약제공의거。방법:대의원2011년1월지2012년12월기간,노년하호흡도감염환자적담급하호흡도분비물중분리출래적1102례양성표본,진행세균감정화약민시험。결과:1102주세균중,혁란음성간균776주,점70.4%,혁란양성구균210주,점19.1%;세균학배양양성전6위적위:동록가단포균、폐염극뢰백균、포만불동간균、대장애희균、금황색포도구균、진균。혁란음성간균대아알배남、두포고동/서파탄、고랍서림/타서파탄민감성교고,균>70%,금황색포도구균대만고매소、리내서알민감성교고,균위100%。결론:노년환자하호흡도감염병원균이혁란음성간균위주,단대다충항균약물산생내약,인차,림상응중시하호흡도감염환자적표본송검솔,근거약민결과합리선용항균약물,사노년하호흡도감염환자조진단조치유。
Objective:to elderly patients with lower respiratory tract infection pathogens distribution and drug resistance monitoring analysis, provide the basis for clinicians rational drug use. Methods:the hospital between January 2011 and December 2011, during the older lower respiratory tract infection in patients with phlegm and separated from the lower respiratory tract secretions of 1102 cases positive specimens, bacteria identification and drug sensitive test. Results:1102 strains of bacteria, gram negative bacil i 776 strains, accounting for 70.4%, 210 strains gram-positive cocci, accounted for 19.1%;Bacteriology to cultivate positive before 6:pseudomonas aeruginosa, klebsiel a pneumoniae, acinetobacter baumanni , e. coli, staphylococcus aureus and fungi. Gram-negative bacil i of imine pei south, cefoperazone/shu batan, piperacil in/he thiazole batan sensitivity is higher, were>70%,Staphylococcus aureus to vancomycin, rina thiazole amine sensitivity is higher, to 100%. Conclusion:elderly patients with lower respiratory tract infection pathogens mainly gram-negative bacil i, but produce drug resistance to many kinds of antimicrobial agents, therefore, should pay at ention to the lower respiratory tract infection in patients with clinical specimens inspection rate, according to the rational use of antimicrobial agents susceptibility results, make the older lower respiratory infection in patients with early diagnosis and cure.
