目的:探讨原发性高血压(EH)患者血压变异(BPV)与颈动脉硬化(CAS)的关系。方法114例EH患者根据颈动脉超声检查结果分为CAS组66例和非CAS组48例,比较两组患者性别、年龄、吸烟、空腹血糖(FBG)、血脂、血肌酐(Cr)、血尿酸(UA)、血压及 BPV的差异,并对CAS相关危险因素进行Logistic回归分析。结果 CAS组患者性别、年龄、吸烟、TC、LDL-C、24 h平均收缩压(24 h SBP)、24 h收缩压标准差(24 h SSD)、24 h舒张压标准差(24 h DSD)均高于非CAS组(P<0.01),HDL-C低于非CAS组(P<0.01),Logistic回归分析显示年龄、吸烟、TC、HDL-C、24 h SBP及24 h SSD均是CAS主要危险因素(P<0.05)。结论 EH患者BPV与CAS密切相关,降低BPV对减少高血压靶器官损害具有重要意义。

목적:탐토원발성고혈압(EH)환자혈압변이(BPV)여경동맥경화(CAS)적관계。방법114례EH환자근거경동맥초성검사결과분위CAS조66례화비CAS조48례,비교량조환자성별、년령、흡연、공복혈당(FBG)、혈지、혈기항(Cr)、혈뇨산(UA)、혈압급 BPV적차이,병대CAS상관위험인소진행Logistic회귀분석。결과 CAS조환자성별、년령、흡연、TC、LDL-C、24 h평균수축압(24 h SBP)、24 h수축압표준차(24 h SSD)、24 h서장압표준차(24 h DSD)균고우비CAS조(P<0.01),HDL-C저우비CAS조(P<0.01),Logistic회귀분석현시년령、흡연、TC、HDL-C、24 h SBP급24 h SSD균시CAS주요위험인소(P<0.05)。결론 EH환자BPV여CAS밀절상관,강저BPV대감소고혈압파기관손해구유중요의의。
Objective To explore the relationship between blood pressure variability(BPV) and carotid atherosclerosis(CAS) in patients with essential hypertension(EH). Methods 114 patients with EH were divided into carotid atherosclerosis group(CAS group, n=66) and non-carotid atherosclerosis group(NCAS group, n=48) by carotid artery ultrasonography. Gender, age, smoking, fasting blood glucose(FBG), blood lipid, serum creatinine(Cr), blood uric acid (UA), blood pressure, and BPV were measured. The related risk factors of CAS were analyzed by multivariate Logistic regression analysis. Results Compared with NCAS group, gender, age, smoking, TC, LDL-C, 24-hour mean systolic blood pressure(24 h SBP), 24-hour systolic blood pressure standard deviation (24 h SSD), and 24-hour diastolic blood pressure standard deviation(24 h DSD) were significantly higher in CAS group(P<0.01), HDL-C was significantly lower in CAS group(P<0.01). Multivariate Logistic regression analysis indicated that age, smoking, TC, HDL-C, 24 h SBP, 24 h SSD were all important influencing factors for CAS(P<0.05). Conclusion BPV is correlated with CAS in EH patients, there are important meaning in reducing the damage of hypertension target organ to decrease the BPV.
