目的:定量分析病人静脉注射对比剂后生成的数字重组影像对射波刀颅骨追踪影像引导方式的影响。资料与方法选取41例采用射波刀治疗颅内肿瘤的病人,采用相同扫描参数行头部平扫CT和增强CT,分别生成基于上述CT的数字重组影像,并依次与病人治疗时采集的实时影像进行配准。增强CT和平扫CT配准后分别生成6个位置参量,对两组影像的各位置参量进行Pearson相关检验,并使用t检验定量分析两组配准结果。结果两组间各位置参量具有高度相关性,各位置参量间的偏差均值在±0.02 mm 之内,增强CT 组和平扫CT组相应位置参量的最大差异为0.3 mm,但该偏差无统计学意义(P>0.05)。结论是否使用对比剂对射波刀进行颅骨追踪的影像引导精度的影响非常小,差异远远小于系统误差,因此临床中该影响可以忽略不计。

Objective To quantitatively evaluate the effect of using digitally reconstructed radiograph (DRR) generated images from contrast-enhanced CT on the accuracy of Cyberknife skull tracking. Methods Forty-one patients with intracranial lesions, planning to receive Cyberknife treatment, were enrolled. Each patient underwent CT with the same scanning parameters before and after administration of contract medium. For each patient 2 sets of DRR images (noncontrast, contrast) were generated as the references for the skull tracking image guided procedure. After image registering between DRR images and patients’ digital images generated by image guided system of Cyberknife, paired t-test was applied on the results. Results The deviations in 6 sets of position were all within ±0.02mm. The max deviation between the two reference groups was 0.3 mm. All of the results have no statistical differences (P>0.05). Conclusion Using contrast medium or not for generating reference has little effect on accuracy of Cyberknife skull tracking.
