目的:研究沙漠干热环境中创伤失血性休克大鼠继发性肺损伤病理改变与肺组织丙二醛(MDA)浓度、总超氧化物歧化酶(T-SOD)活力及环氧化物酶-2(COX-2)mRNA表达量的关系。方法140只雄性SD大鼠随机分为常温环境创伤失血性休克组和干热环境创伤失血性休克组,每组又分别设对照组、休克后0h、0.5h、1h、1.5h、2h、3h 7个亚组,每组10只,建立创伤失血性休克模型后,留取肺灌洗液和组织。观察肺组织病理学改变,检测肺灌洗液总蛋白量和肺组织匀浆MDA浓度、T-SOD活力及COX-2 mRNA表达量的变化。结果病理观察可见干热组各时间点肺病理损伤较常温组病理损伤严重,肺损伤病理学评分较高;干热组肺脏灌洗液总蛋白量高于常温组,峰值出现较早。干热组休克后各时间点MDA浓度较常温组高,而T-SOD活力均较常温组低; COX-2 mRNA表达量干热组休克后各时间点均高于常温组,干热组1.5h时峰值出现,常温组则在2h达到峰值,COX-2 mRNA表达与肺损伤病理学评分呈正相关(P<0.05)。结论在沙漠干热环境中创伤失血性休克继发性肺损伤时,肺脏损伤较常温环境下严重且损伤提前; MDA浓度和T-SOD活力的变化是反映继发性肺损伤的重要指标; COX-2在沙漠干热环境创伤失血性休克继发性肺损伤过程中起重要作用,可能成为药物干预治疗的有效靶点。

목적:연구사막간열배경중창상실혈성휴극대서계발성폐손상병리개변여폐조직병이철(MDA)농도、총초양화물기화매(T-SOD)활력급배양화물매-2(COX-2)mRNA표체량적관계。방법140지웅성SD대서수궤분위상온배경창상실혈성휴극조화간열배경창상실혈성휴극조,매조우분별설대조조、휴극후0h、0.5h、1h、1.5h、2h、3h 7개아조,매조10지,건립창상실혈성휴극모형후,류취폐관세액화조직。관찰폐조직병이학개변,검측폐관세액총단백량화폐조직균장MDA농도、T-SOD활력급COX-2 mRNA표체량적변화。결과병리관찰가견간열조각시간점폐병리손상교상온조병리손상엄중,폐손상병이학평분교고;간열조폐장관세액총단백량고우상온조,봉치출현교조。간열조휴극후각시간점MDA농도교상온조고,이T-SOD활력균교상온조저; COX-2 mRNA표체량간열조휴극후각시간점균고우상온조,간열조1.5h시봉치출현,상온조칙재2h체도봉치,COX-2 mRNA표체여폐손상병이학평분정정상관(P<0.05)。결론재사막간열배경중창상실혈성휴극계발성폐손상시,폐장손상교상온배경하엄중차손상제전; MDA농도화T-SOD활력적변화시반영계발성폐손상적중요지표; COX-2재사막간열배경창상실혈성휴극계발성폐손상과정중기중요작용,가능성위약물간예치료적유효파점。
Objective To study the relationship among the changes of lung tissue malondialdehyde ( MDA) concentration ,total superoxide dismutase ( T-SOD) activity and COX-2 mRNA expression levels of traumatic hemor-rhagic shock rats and their correlations to the secondary lung injury in the dry heat environment of desert .Methods Totally 140 male SD rats with traumatic hemorrhagic shock were randomly divided into the dry heat environment group and the normal temperature environment group .Each group was randomly divided into 7 subgroups:the con-trol group,0 hour group,0.5 hours group,1 hour group,1.5 hours group,2 hours group,3 hours group,10 rats in each subgroup .After the establishment of traumatic hemorrhagic shock rat models in each environment ,at the corre-sponding time points ,specimens from the bronchoalveolar lavage fluid ( BALF) and lung tissue were harvested .The optical microscopy was performed to examine the pathological changes in lung ,the total protein of BALF was meas-ured,the concentrations of MDA ,T-SOD and COX-2 mRNA in lung tissue were detected .Results The pathologi-cal injury was more serious and the histopathology score for lung injury was higher in the dry heat environment group than in the corresponding normal temperature environment group .The MDA concentration at each time point in the dry heat environment group was higher than that of the corresponding normal temperature environment group .How-ever,the T-SOD activity was lower than that of the normal temperature environment group .The expressions of COX-2 mRNA were higher in dry heat environment group than the corresponding normal temperature environment group . COX-2 mRNA expression reached its peak at 1.5 hours in the dry heat environment group ,however,COX-2 mRNA reached its peak at 2 hours in the normal temperature environment group .The expressions of COX-2 mRNA were positively correlated with lung injury scores ( P<0.05 ) . Conclusion The secondary lung injuries of traumatic hemorrhagic shock rats are more serious in dry heat environment than in normal temperature environment .MDA and T-SOD are important index reflecting secondary lung injury .COX-2 mRNA plays an important role in the secondary injury of traumatic hemorrhagic shock rats in the dry heat environment of desert .COX-2 may be an effective target in the treatment of traumatic hemorrhagic shock .
