目的:评价MR扩散加权成像(DWI)在前列腺良、恶性病变鉴别诊断中的价值。材料与方法共47例患者进行MRI检查,其中良性前列腺增生29例,前列腺癌18例。检查方法包括常规T1WI、T2WI、DWI和脂肪抑制成像。所有病例经穿刺活检或手术证实。观察前列腺大小、病变位置、信号特点及肿瘤侵犯情况,对DWI、ADC数据行定量分析。结果 MRI显示18例恶性肿瘤患者中,发生于外周带13例,表现为T2WI外周带内结节状、片状异常低信号区;8例位于中央带或移行带。DWI对前列腺癌的诊断敏感度为81.07%、特异度为78.01%、准确率为80.11%。结论联合应用DWI成像方法可以提高MR对前列腺癌诊断的准确率。

목적:평개MR확산가권성상(DWI)재전렬선량、악성병변감별진단중적개치。재료여방법공47례환자진행MRI검사,기중량성전렬선증생29례,전렬선암18례。검사방법포괄상규T1WI、T2WI、DWI화지방억제성상。소유병례경천자활검혹수술증실。관찰전렬선대소、병변위치、신호특점급종류침범정황,대DWI、ADC수거행정량분석。결과 MRI현시18례악성종류환자중,발생우외주대13례,표현위T2WI외주대내결절상、편상이상저신호구;8례위우중앙대혹이행대。DWI대전렬선암적진단민감도위81.07%、특이도위78.01%、준학솔위80.11%。결론연합응용DWI성상방법가이제고MR대전렬선암진단적준학솔。
Objective:To evaluate diffusion weighted imaging (DWI) in the differential diagnosis of prostate benign and malignant lesions. Materials and Methods:Of 47 cases, 29 patients with benign prostatic hyperplasia and 18 cases of prostate cancer patients underwent magnetic resonance imaging check, scanning sequences included conventional T1WI, T2WI, DWI and fat suppression imaging. All cases were conifrmed by biopsy or surgery. Prostate size, lesion location, signal characteristics and tumor invasion situation were observed. The quantitative analysis of ADC and DWI was applied. Results:13 of 18 cases with prostatic cancer located in the peripheral zone, the performance were peripheral zone nodules, lfaky low signal area in T2WI;8 cases were in the central zone or transition zone. The sensitivity, speciifcity, accuracy rate of DWI for the diagnosis of prostate cancer were 81.07%, 78.01%,80.11%, separately. Conclusion:DWI can improve the accuracy rate of diagnosis of prostate cancer.
