目的：动态监测早孕期血清CA125及孕酮水平，探讨其在预测早期异位妊娠结局的价值。方法选取2011年3月至2013年2月于该院门诊正常妊娠及疑为异位妊娠或先兆流产而入院的80例患者作为研究对象，按妊娠结局分为正常妊娠分娩组20例，先兆流产保胎成功分娩组20例，先兆流产发展为稽留流产组20例，异位妊娠组20例，对所有病例进行血清CA125及孕酮动态监测，并进行统计学分析与处理。结果先兆流产保胎成功分娩组孕酮水平高于稽留流产组，明显高于异位妊娠组，均有明显统计学意义（ P ＜0貂．05）。早期妊娠阶段，正常妊娠分娩组、先兆流产保胎成功分娩组、先兆流产保胎失败组，孕酮水平差异无统计学意义（ P ＞0．05）。早期妊娠阶段，4组血清CA125平均水平均高于正常值且呈缓慢上升趋势，正常妊娠分娩组、先兆流产保胎成功分娩组呈缓慢上升趋势，两者无明显统计学意义（ P ＞0．05），而先兆流产发展为稽留流产组及异位妊娠组的CA125水平高于前2组（ P ＜0．05），但2组无明显统计学意义（ P ＞0．05），但分别与正常妊娠分娩组及及保胎成功分娩组相比，均有统计学意义（ P ＜0．05）。结论动态监测血清孕酮联合CA125值，能够更准确的预测早期异位妊娠结局。
목적：동태감측조잉기혈청CA125급잉동수평，탐토기재예측조기이위임신결국적개치。방법선취2011년3월지2013년2월우해원문진정상임신급의위이위임신혹선조유산이입원적80례환자작위연구대상，안임신결국분위정상임신분면조20례，선조유산보태성공분면조20례，선조유산발전위계류유산조20례，이위임신조20례，대소유병례진행혈청CA125급잉동동태감측，병진행통계학분석여처리。결과선조유산보태성공분면조잉동수평고우계류유산조，명현고우이위임신조，균유명현통계학의의（ P ＜0초．05）。조기임신계단，정상임신분면조、선조유산보태성공분면조、선조유산보태실패조，잉동수평차이무통계학의의（ P ＞0．05）。조기임신계단，4조혈청CA125평균수평균고우정상치차정완만상승추세，정상임신분면조、선조유산보태성공분면조정완만상승추세，량자무명현통계학의의（ P ＞0．05），이선조유산발전위계류유산조급이위임신조적CA125수평고우전2조（ P ＜0．05），단2조무명현통계학의의（ P ＞0．05），단분별여정상임신분면조급급보태성공분면조상비，균유통계학의의（ P ＜0．05）。결론동태감측혈청잉동연합CA125치，능구경준학적예측조기이위임신결국。
Objective To detect dynamically the serum levels of progesterone and CA 125 in order to explore their significance in predicting the outcome of early ectopic pregnancy .Methods Eighty pregnant women who were suspected for threatened abortion or ectopic pregnancy between 2011 March to 2013 February during clinic service were enrolled in the study.These women were divided into four groups according to pregnancy outcome ;normal pregnancy group ( n =20), threatened abortion group ( n =20 ) , missed abortion group ( n =20 ) and ectopic pregnancy group ( n =20 ) .The serum levels of progesterone and CA 125 were detect dynamically in all the subjects , and the results were treated and analyzed statistically.Results The serum levels of progesterone in threatened abortion group were significantly higher than those of missed abortion group ( P <0.05 ).During early pregnancy there were no significant differences in serum levels of progesterone among normal pregnancy group,threatened abortion group and missed abortion group ( P >0.05),however,the serm levels of CA125 were obviously higher than normal levels in the 4 groups,with slow increase tendency ,but there were no significant differences between normal pregnancy group and threatened abortion group ( P >0 .05 ) .The serum levels of CA125 in missed abortion group and ectopic pregnancy group were significantly higher than those in normal pregnancy group and threatened abortion group ( P <0 .05 ) ,however ,there were no significant differences between missed abortion group and ectopic pregnancy group ( P >0.05).However the CA125 levels in threatened abortion group and ectopic pregnancy group were significantly higher than those of the other two groups ( P <0.05 ) .Conclusion The combined dynamic detection of serum levels of progesterone and CA 125 can predict the outcome of early ectopic pregnancy more accurately .
