目的:探讨超声造影在评价下肢动脉硬化闭塞症( Lower extremity arterial disease ,LEAD )的骨骼肌灌注障碍的应用价值。方法收集80例正常对照组和80例LEAD患者,用超声造影观察小腿骨骼肌运动前后血流灌注情况,运用数学模型分析时间-强度曲线,得出灌注参数达峰时间( time to peak intensity ,TTP)。结果时间-强度曲线显示在静息时对照组TTP 38 s(19~65 s),LEAD组TTP 84 s(39~142 s),2组比较差异有统计学意义( P <0 f.01),小腿运动试验后,正常对照组TTP 11 s(9~22 s),LEAD组TTP 52 s(27~105 s),2组比较差异有统计学意义( P <0.01),年龄校正后,2组比较差异有统计学意义( P <0.01)。 TTP和踝肱指数、年龄均无明显相关性( P >0.05)。结论超声造影是一种评价骨骼肌灌注障碍的新型的诊断方法。

Objective To explore the application value of contrast-enhanced ultrasound ( CEUS ) in the diagnosis of lower extremity arterial disease ( LEAD) .Met hods Ultrasound contrast agent was injected into peripheral vein of 80 healthy subjects and 80 patients with LEAD .The condition of blood flow perfusion in calf muscle was detected by contrast-enhanced ultrasound.Time intensity curve (TIC) was analyzed by mathematical model to get the time to peak intensity (TTP).Results TIC revealed TTP in patients with LEAD was 84s(39~142s)at rest period,which was 38s(19~65s in control group,there was a significant difference between LEAD group and control group ( P <0.01).After leg exercise test,TTP in control group was 11s(9~22s),however,which was 52s(27 ~105s) in LEAD group,there was a significant difference between LEAD group and control group ( P <0.01).After age adjustment there was a significant difference between LEAD group and control group ( P <0.01).However TTP was not correlated to patient's age and ankle brachium index .Conclusion CEUS is an novel diagnostic method for evaluating perfusion disorder of skeletal muscles .
