目的：观察早发性乳腺癌（EOBC）的超声声像图表现特点，探讨超声对EOBC的诊断价值。方法以年龄小于35岁的60例EOBC患者为观察组，年龄大于35岁的乳腺癌患者50例为对照组，两组均行乳腺彩色多普勒超声检查。结果60例EOBC患者有59例术前超声诊断为乳腺癌，1例误诊为乳腺炎性假瘤。浸润性导管癌、小叶癌、导管内癌的超声多表现为直接征象，髓样癌多表现为间接征象。观察组与对照组乳腺癌患者均以团块型为主，分别占到72．9％和87．5％，两组比较差异具有统计学意义（χ2＝6．293，P＜0.05）。两组彩色多普勒血流丰富程度、病灶形态、边界清晰度、有无高回声晕、钙化灶和毛刺征或蟹足征差异均无统计学意义（P＞0．05）；观察组内部回声不均匀、后方衰减程度与对照组差异均有统计学意义（P＜0.05）。观察组EOBC患者癌灶前后径和纵横之比均明显小于对照组，差异具有统计学意义（t＝6．720、9.285，P＜0．05）；横径明显高于对照组（t＝34．962，P＜0．05）。结论早发性乳腺癌在彩色多普勒超声诊断中具有横径大而纵横比小、多见内部回声不均匀及后方衰减较少的特征性表现，对筛查年轻女性乳腺癌具有重要意义。
목적：관찰조발성유선암（EOBC）적초성성상도표현특점，탐토초성대EOBC적진단개치。방법이년령소우35세적60례EOBC환자위관찰조，년령대우35세적유선암환자50례위대조조，량조균행유선채색다보륵초성검사。결과60례EOBC환자유59례술전초성진단위유선암，1례오진위유선염성가류。침윤성도관암、소협암、도관내암적초성다표현위직접정상，수양암다표현위간접정상。관찰조여대조조유선암환자균이단괴형위주，분별점도72．9％화87．5％，량조비교차이구유통계학의의（χ2＝6．293，P＜0.05）。량조채색다보륵혈류봉부정도、병조형태、변계청석도、유무고회성훈、개화조화모자정혹해족정차이균무통계학의의（P＞0．05）；관찰조내부회성불균균、후방쇠감정도여대조조차이균유통계학의의（P＜0.05）。관찰조EOBC환자암조전후경화종횡지비균명현소우대조조，차이구유통계학의의（t＝6．720、9.285，P＜0．05）；횡경명현고우대조조（t＝34．962，P＜0．05）。결론조발성유선암재채색다보륵초성진단중구유횡경대이종횡비소、다견내부회성불균균급후방쇠감교소적특정성표현，대사사년경녀성유선암구유중요의의。
Objective To observe the early onset breast cancer ( EOBC) sonographic performance character-istics and explore the diagnostic value of ultrasonography in EOBC .Methods 60 cases EOBC patients younger than 35 years old were seclected as the observation group .Older than 35 years of 50 patients with breast cancer were se-clected as the control group .Two groups were underwent breast color Doppler ultrasound .Results 59 cases in 60 pa-tients with preoperative ultrasound diagnosis of breast cancer ,one case was misdiagnosed as mastitis pseudotumor infil-trating ductal carcinoma ,lobular carcinoma ,ductal carcinoma in ultrasound showed more direct signs ,medullary carci-noma showed more indirect signs .Clumps were main types of breast cancer of the observation group and control group , respectively,accounted for 72.9% and 87.5%,the difference was statistically significant (χ2 =6.293,P<0.05). Two groups of color Doppler richness , lesion shape , boundary definition , without hyperechoic halo , calcification and glitches levy or levy Crab compared were not significant (P>0.05);Compared with the control group ,heterogeneous internal echo,rear attenuation of observation group had significant differences (P<0.05).Foci were observed group EOBC anteroposterior diameter and aspect ratio were significantly lower than that of the control group ,the difference was statistically significant(t =6.720,9.285,all P <0.05);diameter was significantly higher (t =34.962,P <0.05).Conclusion Early-onset breast cancer at diagnosis with color Doppler ultrasound large diameter aspect ratio is small,and the rear common heterogeneous internal echo attenuation less characteristic features ,screening younger women for breast cancer is important .
