目的：探讨贲门癌患者围手术期心血管并发症的发病原因及预防处理方法。方法回顾性分析接受手术治疗的贲门癌患者1377例，观察心血管并发症与心肺功能、年龄、手术方式的关系。结果术中、术后心血管并发症112例，发生率为8．1％。其中心律失常多发。老年组术后心血管并发症发生率18.7％，较一般组显著偏高（χ2＝93．100，P＜0．05）。术前心肺功能异常的患者术后心血管并发症发生率20.2％，较心肺功能正常组（3．8％）显著偏高（χ2＝82．167，P＜0．05）。弓上吻合术式组术后发生心血管并发症的发生率显著高于其他术式组（χ2＝60．063，P＜0．05）。结论术前、术中对患者应用常规心电图、动态心电图、运动平板和心电监护等技术检测患者心电异常反应，并能够及时对围手术期心血管危险因素采取有效干预，可以积极改善患者血液流变性，减少术中、术后并发症的发生。
목적：탐토분문암환자위수술기심혈관병발증적발병원인급예방처리방법。방법회고성분석접수수술치료적분문암환자1377례，관찰심혈관병발증여심폐공능、년령、수술방식적관계。결과술중、술후심혈관병발증112례，발생솔위8．1％。기중심률실상다발。노년조술후심혈관병발증발생솔18.7％，교일반조현저편고（χ2＝93．100，P＜0．05）。술전심폐공능이상적환자술후심혈관병발증발생솔20.2％，교심폐공능정상조（3．8％）현저편고（χ2＝82．167，P＜0．05）。궁상문합술식조술후발생심혈관병발증적발생솔현저고우기타술식조（χ2＝60．063，P＜0．05）。결론술전、술중대환자응용상규심전도、동태심전도、운동평판화심전감호등기술검측환자심전이상반응，병능구급시대위수술기심혈관위험인소채취유효간예，가이적겁개선환자혈액류변성，감소술중、술후병발증적발생。
Objective To investigate the cause and treatment methods of cardiovascular complications in e -sophageal and cardiac perioperative .Methods 1 377 patients who underwent esophageal and cardiac surgery were chosen for the study .The relationship between heart and lung function ,age,and other factors with cardiovascular surgi-cal complications were retrospectively analyzed .Results In and after surgery ,there were 112 cases of cardiovascular disease,the incidence rate was 8.1%.The arrhythmia symptoms were the major complications .The incidence of post-operative cardiovascular complications (18.7%) in elderly patients,which was significantly higher than non-elderly group(χ2 =93.100,P<0.05).Preoperative cardiopulmonary dysfunction in patients with postoperative cardiovascu -lar morbidity(20.2%)was significantly higher than that of the normal group (3.8%)(χ2 =82.167,P<0.05).Arch anastomosis group after the onset of cardiovascular disease was significantly higher than that of other surgical patients (χ2 =60.063,P<0.05).Conclusion ECG,Holter monitoring and exercise treadmill ECG technology applied to preoperative and intraoperative patients detects abnormal reaction of the patients ,which has the ability to take effective intervention timely for perioperative cardiovascular risk factors ,can actively improve blood rheology ,reduce postopera-tive and intraoperative complications .
