目的：探讨(1-3)-β-D葡聚糖和半乳甘露聚糖抗原检测在侵袭性真菌病诊断中的临床意义。方法回顾性研究苏州大学附属第一医院临床疑诊侵袭性真菌病的住院患者231例。根据诊断标准确定是否诊断侵袭性真菌病，分析血清(1-3)-β-D葡聚糖（G试验）和血清半乳甘露聚糖（GM试验）的敏感度和特异度，以及两者联合应用后诊断的敏感度和特异度。结果G试验灵敏度为50.0%，特异度为91.5%；GM试验灵敏度为70.2%，特异度为83.7%。两者联合试验时，其灵敏度和特异度分别为92.7%和78.9%。结论 G试验和GM试验均对侵袭性真菌病具有诊断价值，但两者联合应用可使其应用价值进一步提高。
목적：탐토(1-3)-β-D포취당화반유감로취당항원검측재침습성진균병진단중적림상의의。방법회고성연구소주대학부속제일의원림상의진침습성진균병적주원환자231례。근거진단표준학정시부진단침습성진균병，분석혈청(1-3)-β-D포취당（G시험）화혈청반유감로취당（GM시험）적민감도화특이도，이급량자연합응용후진단적민감도화특이도。결과G시험령민도위50.0%，특이도위91.5%；GM시험령민도위70.2%，특이도위83.7%。량자연합시험시，기령민도화특이도분별위92.7%화78.9%。결론 G시험화GM시험균대침습성진균병구유진단개치，단량자연합응용가사기응용개치진일보제고。
Objective To evaluate the (1-3)-β-D Glucan and Galactomannan for diagnosis of invasive fungal disease (IFD). Methods A retrospective study was performed on 231 cases of hospitalized patients of the First Affiliated Hospital of Soochow University who were at risk of IFD. The diagnosis of IFD was according to revised definitions. Galactomannan enzyme immunoassay (GM test) and (1-3)-β-D glucan assay (G test) were analyzed of sensitivity, specificity, and concordance rate, respectively. Two tests were combined to evaluate the changes of sensitivity, specificity, and concordance rate. Results The sensitivity and specificity were 50.0%, 91.5%in G test;70.2%, 83.7%in GM test;and 92.7%, 78.9%in combined utility of two tests. Conclusion The G test and GM test were both useful in diagnosis of IFD. The better sensitivity and concordance rate might be acquired by combining these two tests.
