目的:探讨64层螺旋C T低剂量对比剂血管成像在骨盆骨折动脉损伤诊断中的可行性。方法对15例经X线确诊不稳定性骨盆骨折患者行盆腔CTA检查,增强扫描采用 Triger技术,对比剂60ml追加生理盐水40ml ,注射速率4ml/s。由2名有经验的放射科医师独立评价髂内动脉及其分支的显示程度,测量髂内动脉内对比剂浓度,并分析骨盆骨折周围血管走行及损伤情况。结果①所有病例均清晰显示双侧髂内动脉及其分支,髂内动脉内的对比剂浓度维持在较高水平;②21处骨折周围血肿内见对比剂外渗,3例患者显示髂外动脉受压移位。结论64层C T低剂量对比剂血管成像可以满足骨盆骨折动脉损伤的诊断需要。

목적:탐토64층라선C T저제량대비제혈관성상재골분골절동맥손상진단중적가행성。방법대15례경X선학진불은정성골분골절환자행분강CTA검사,증강소묘채용 Triger기술,대비제60ml추가생리염수40ml ,주사속솔4ml/s。유2명유경험적방사과의사독립평개가내동맥급기분지적현시정도,측량가내동맥내대비제농도,병분석골분골절주위혈관주행급손상정황。결과①소유병례균청석현시쌍측가내동맥급기분지,가내동맥내적대비제농도유지재교고수평;②21처골절주위혈종내견대비제외삼,3례환자현시가외동맥수압이위。결론64층C T저제량대비제혈관성상가이만족골분골절동맥손상적진단수요。
Objective To investigate the feasibility of 64-slice CT angiography with low dose contrast media in pelvic ar-terial injuries with pelvic fractures .Methods 15 patients with unstable pelvic fractures underwent CTA examination . Contrast material of 60 ml and saline 40 ml were administered at a rate of 4 .0 ml/s using trigger technology .The demon-stration of the internal iliac artery and its branches ,especially the injured arterials around pelvic fractures ,were evaluated by two radiologists ,independently .Intravascular CT attenuation values were recorded and assessed .Results ① In all ca-ses ,the bilateral internal iliac arteries and their branches were shown well ,and the intravascular CT attenuation values were all high enough ;② The extravasation of contrast material within a hematoma were found around 21 fracture sites . The external iliac artery was compressed and shifted in three patients .Conclusion It is feasible to use 64-slice CT angiog-raphy with low dose contrast media in pelvic arterial injuries .
