[目的]运用循证护理方法，为鼻饲老年病人堵管并发症的预防寻求合理的护理方案。[方法]在充分评估堵管的可能相关因素后，根据PICO 原则，提出临床问题，检索Cochrane 图书馆、SCI、MEDLINE及CBM数据库，进行文献分析，筛选出有效护理措施，形成循证护理方案。同时，结合护理人员能力和病人实际情况，将以上方案用于120例鼻饲病人，并观察效果。[结果]经过检索，最终纳入文献17篇，包括1篇系统评价(SR)、14篇随机对照试验(RCT)和2篇综述(Re-view)。循证护理方案包括营养液输注方式、日常管道维护、管道的选择、堵管后处理。实施循证护理方案后，堵管发生率为6.7%。[结论]对于实施肠内营养的鼻饲病人，运用循证护理方法，寻求科学合理的有效护理方案，减少了堵管发生率，保证了护理质量。

Obj ective:To use evidence based nursing method,to seek for a reasonable care scheme for prevention of nasal feeding tube occlusion compli-cations of elderly patients.Methods:After fully evaluation on possible related factors of tube occlusion,according to the principle of PICO the clinical ques-tions were put forward.The Cochrane library,SCI,MEDLINE and CBM da-tabase were retrieved and literature analysis was carried out and effective nursing measures were screened.Evidence based nursing program was formed.At the same time,in combination with the patients’practical situa-tion and ability of nursing staff,the above program used in 120 cases of nasal feeding patients.And the effect were observe.Results:After retrieval,1 7 lit-eratures were eventually incorporated into,including 1 article system evalua-tion(SR),14 randomized controlled trial (RCT)and 2 reviews (Review).Ev-idence based nursing programs included infusion method of nutrient fluids, daily pipeline maintained,selection of tube occlusion and treatment after.Af-ter the implementation of evidence based nursing care programs,tube occlu-sion rate was 6 .7%.Conclusion:For patients with nasogastric enteral nutri-tion,the evidence based nursing was used.It can seek scientific and rational and effective care programs.It can reduce the incidence of tube occlusion and ensure the quality of care.
