目的研究腕管综合征的手术治疗与非手术治疗的效果及并发症,及如何更合理地使用这两种方法.方法 84例腕管综合征,轻度型21例,中度型48例,重度型15例.非手术治疗45例,手术治疗39例.结果非手术治疗对轻度型、中度型病例的近期有效率分别为90.48％、85.00％,2～3年后部分轻、中度型病例的非手术治疗不能阻断病情发展,改手术治疗(5例).重度型病例手术治疗的效果较好.结论轻、中度型腕管综合征可首选非手术治疗,疗效不佳再行手术治疗.重度型病例选择手术治疗.

Objective To analyze the effect and complications of the surgical treatment and non-surgical treatment of carpal tunnel syndrome (CTS),and discuss the rationale of the application of both methods.Methods A total of 84 cases of carpal tunnel syndrome were analyzed,among which 21 were mild CTS,48were intermediate CTS,and 15 were severe CTS.39 cases were surgically treated,while 45 cases were treated with non-surgical methods. Results The short term efficiency of non-surgical treatment for mild and intermediate cases were 90.48％ and 85.00％,respectively.After 2 to 3 years,some patients who had nonsurgical treatment for mild and intermediate CTS had progression of symptoms and had to resort to surgical treatment (5 cases).The effect of surgical treatment for severe cases was better than that of non-surgical treatment.Conclusion Non-surgical treatment is the first choice for mild and intermediate CTS.If nonsurgical treatment is ineffective,surgical treatment can be chosen.Severe cases should be surgically treated.
