目的：观察壳聚糖宫颈抗菌膜在宫颈环形电切术（LEEP）治疗后，减少创面感染、促使创面愈合及预防术后出血的临床效果，寻求预防宫颈环切术后并发症的有效方法。方法选择进行LEEP术治疗的宫颈疾病患者200例，随机分成两组，LEEP术后采用壳聚糖宫颈抗菌膜创面上药者100例作为治疗组，LEEP术后自然修复100例作为对照组。对照组发生出血量多时用云南白药和甲硝唑药粉混合局部上药止血。结果观察组阴道出血量、流液量及流血持续时间少于对照组（P＜0.05）；观察组白带异常情况明显少于对照组（P＜0.05）；创面愈合总有效率两组差异无统计学意义，术后2个月和3个月观察痊愈率，观察组与对照组比较差异有统计学意义（P＜0.05）。结论壳聚糖宫颈抗菌膜具有局部抗菌、止血及促进术后创面愈合作用，使宫颈愈合时间缩短，减少创面感染、渗出、出血，以及LEEP术后并发症。
목적：관찰각취당궁경항균막재궁경배형전절술（LEEP）치료후，감소창면감염、촉사창면유합급예방술후출혈적림상효과，심구예방궁경배절술후병발증적유효방법。방법선택진행LEEP술치료적궁경질병환자200례，수궤분성량조，LEEP술후채용각취당궁경항균막창면상약자100례작위치료조，LEEP술후자연수복100례작위대조조。대조조발생출혈량다시용운남백약화갑초서약분혼합국부상약지혈。결과관찰조음도출혈량、류액량급류혈지속시간소우대조조（P＜0.05）；관찰조백대이상정황명현소우대조조（P＜0.05）；창면유합총유효솔량조차이무통계학의의，술후2개월화3개월관찰전유솔，관찰조여대조조비교차이유통계학의의（P＜0.05）。결론각취당궁경항균막구유국부항균、지혈급촉진술후창면유합작용，사궁경유합시간축단，감소창면감염、삼출、출혈，이급LEEP술후병발증。
Objective With the application of cervical chitosan antibacterial film, this study was aimed to explore an effective method to prevent the postoperative complications after loop electrosurgical excision procedure(LEEP)by observing the clinical effect to reduce wound infection, promote wound healing and prevent postoperative bleeding. Methods 200 cases of patients with chronic cervicitis needing LEEP were divied into two groups randomly. After LEEP, cervical chitosan antimicrobial film was applied in 100 cases in the treatment group, LEEP after the natural repair in 100 cases as control group. The control group bleeding time with Yunnan Baiyao and metronidazole powder mixed medicinehemostatic. Results The treatment group of vaginal bleeding, vaginal fluid, and the duration of vaginal bleeding was less than that of the control group(P<0.05 ).The treated group leucorrhea abnormal conditions had obvious difference(P<0.05). Wound healing, the total efficiency of two groups had no statistical significance, but the cure rate after 2months compared with 3 months to observe the treatment group and the control group had statistical significance(P<0.05 ). Conclusion Chitosan cervical antibacterial film with topical anti bacterial, hemostatic and wound healing promoting effects of postoper ativecervical, the healing time was shortened, reducing wound infection, oozing, bleeding, and postoperative complications.
