目的:探讨超声在婴幼儿发育性髋关节发育不良(DDH)诊断中的作用,并对比研究蒙古族与汉族婴幼儿DDH发病率的情况。方法6个月以下婴幼儿332例,共664个髋关节,二维超声应用Graf法检查髋关节,将髋关节分为5型;并辅以四维超声观察关节整体情况。结果Ⅳ型完全脱位5个,Ⅲ型半脱位19个,Ⅱ型发育不良28个,骨性髋臼缘稍钝29个,Ⅰ型583个;汉族与蒙古族DDH发生率差异无统计学意义;女性DDH发病率高于男性(P<0.05),左侧DDH发病率高于右侧(P<0.05)。二维超声与四维超声对髋关节发育情况的检查高度一致。结论蒙古族与汉族DDH发生率无明显族群差异,超声诊断DDH具有简单、安全的优点,更适合DDH的早期筛查。
目的:探討超聲在嬰幼兒髮育性髖關節髮育不良(DDH)診斷中的作用,併對比研究矇古族與漢族嬰幼兒DDH髮病率的情況。方法6箇月以下嬰幼兒332例,共664箇髖關節,二維超聲應用Graf法檢查髖關節,將髖關節分為5型;併輔以四維超聲觀察關節整體情況。結果Ⅳ型完全脫位5箇,Ⅲ型半脫位19箇,Ⅱ型髮育不良28箇,骨性髖臼緣稍鈍29箇,Ⅰ型583箇;漢族與矇古族DDH髮生率差異無統計學意義;女性DDH髮病率高于男性(P<0.05),左側DDH髮病率高于右側(P<0.05)。二維超聲與四維超聲對髖關節髮育情況的檢查高度一緻。結論矇古族與漢族DDH髮生率無明顯族群差異,超聲診斷DDH具有簡單、安全的優點,更適閤DDH的早期篩查。
목적:탐토초성재영유인발육성관관절발육불량(DDH)진단중적작용,병대비연구몽고족여한족영유인DDH발병솔적정황。방법6개월이하영유인332례,공664개관관절,이유초성응용Graf법검사관관절,장관관절분위5형;병보이사유초성관찰관절정체정황。결과Ⅳ형완전탈위5개,Ⅲ형반탈위19개,Ⅱ형발육불량28개,골성관구연초둔29개,Ⅰ형583개;한족여몽고족DDH발생솔차이무통계학의의;녀성DDH발병솔고우남성(P<0.05),좌측DDH발병솔고우우측(P<0.05)。이유초성여사유초성대관관절발육정황적검사고도일치。결론몽고족여한족DDH발생솔무명현족군차이,초성진단DDH구유간단、안전적우점,경괄합DDH적조기사사。
Objective To explore the incidence of developmental dysplasia of hip (DDH) in the Mongol nationality and the Han nationality infants and the role of ultrasonography in diagnosis of DDH. Methods Three hundred and thirty-two infants who were under six months and six hundred and sixty-four hip joints were examined using 2D ultrasonic scanning Graf method. The hip joints were divided into 5 types,and then examined for overall situation using 4D ultrasonic scanning. Results Five hundred and eighty-three hip joints were normal,29 acetabular margin slightly obtuse,28 dysplasia Ⅱ type, 19 subluxation Ⅲ type and 5 dislocation Ⅳtype. There was no significant difference in DDH incidence between the Mongol nationality and the Han nationality infants .The incidence of DDH was higher on the left than on the right,female higher than male (P < 0.05). Conclusion There is no significant difference in DDH incidence between the Mongol nationality and the Han nationality infants. Ultrasonography is a simple and safe method for screening DDH in early stage.
