目的:探讨血浆氨基末端脑钠肽前体(NT-proBNP)与冠心病(CHD)患者冠状动脉病变程度的相关性。方法选择2011~2013年临床拟诊为CHD患者120例,按冠状动脉CT血管造影(CTA)结果分为阳性组(70例)和阴性组(50例),阳性组按冠状动脉病变程度分为单支病变组(24例)、双支病变组(25例)和多支病变组(21例)。结果冠状动脉CTA阴性组患者血浆NT-proBNP水平为(389.4±115.9)pg/mL;冠状动脉CTA 阳性组患者血浆NT-proBNP 水平为(881.6±670.6)pg/mL,二者比较,差异有统计学意义(t=10.5,P<0.01)。冠状动脉CTA阳性组随着冠状动脉病变的严重程度增加,其各组血浆NT-proBNP水平逐渐增高。冠状动脉CTA阳性组各亚组即单支病变组与双支病变组之间、双支病变与多支病变组之间、单支病变与多支病变组NT-proBNP水平比较,差异均有统计学意义(t=8.9、7.5、8.5,P<0.05或0.01)。结论 CHD患者血浆NT-proBNP水平与疾病的严重程度有一定相关性,且随着冠状动脉病变程度的增加,其血浆NT-proBNP水平逐渐增加。

목적:탐토혈장안기말단뇌납태전체(NT-proBNP)여관심병(CHD)환자관상동맥병변정도적상관성。방법선택2011~2013년림상의진위CHD환자120례,안관상동맥CT혈관조영(CTA)결과분위양성조(70례)화음성조(50례),양성조안관상동맥병변정도분위단지병변조(24례)、쌍지병변조(25례)화다지병변조(21례)。결과관상동맥CTA음성조환자혈장NT-proBNP수평위(389.4±115.9)pg/mL;관상동맥CTA 양성조환자혈장NT-proBNP 수평위(881.6±670.6)pg/mL,이자비교,차이유통계학의의(t=10.5,P<0.01)。관상동맥CTA양성조수착관상동맥병변적엄중정도증가,기각조혈장NT-proBNP수평축점증고。관상동맥CTA양성조각아조즉단지병변조여쌍지병변조지간、쌍지병변여다지병변조지간、단지병변여다지병변조NT-proBNP수평비교,차이균유통계학의의(t=8.9、7.5、8.5,P<0.05혹0.01)。결론 CHD환자혈장NT-proBNP수평여질병적엄중정도유일정상관성,차수착관상동맥병변정도적증가,기혈장NT-proBNP수평축점증가。
Objective To investigate the relationship between plasma n-terminal pro-brain natriuretic (NT-proBNP) peptide and severity of coronary artery lesions in patients with coronary heart diseases (CHD). Methods A total of 120 patients with suspected CHD in clinic were divided into positive group(n=70) and negative group(n=50) according to the results of coro-nary computed tomography angioplasty(CTA). The positive group was divided into three sub-groups:single vessel disease group (n=24),double vessel disease group(n=25) and multiple vessel group(n=21). Results The level of plasma NT-proBNP was (389.4±115.9)pg/mL in negative group and (881.6±670.6)pg/mL in positive group,and the difference between the two groups had statistical significance (t=10.5,P<0.01). With the increase of severity,the level of NT-proBNP in three sub-groups increased. The comparison of NT-proBNP level in the three sub-groups (single vessel group,double vessel group and multiple vessel group) of positive group had statistically significant difference(t=8.9,7.5,8.5;P<0.05 or 0.01). Conclusion The plasma concentration of NT-proBNP in patients with CHD has certain relevance with the severity of the disease ,and the plasma NT-proBNP level increases with the degree of the coronary artery lesions.
