目的：调查糖尿病患者血糖达标情况，分析全科医疗服务。方法：采取入户面对面问卷调查，用Logistic等方法分析结果。结果：血压、药物治疗、糖尿病认知和糖化血红蛋白，是影响社区糖尿病管理全面达标的主要因素，也是全科医生的管理重点。结论：社区糖尿病防治管理可试行全科团队服务，固定医患关系，达到诊疗个体化，促进就诊，治疗与转诊的依从性，有效控制患者血糖。
목적：조사당뇨병환자혈당체표정황，분석전과의료복무。방법：채취입호면대면문권조사，용Logistic등방법분석결과。결과：혈압、약물치료、당뇨병인지화당화혈홍단백，시영향사구당뇨병관리전면체표적주요인소，야시전과의생적관리중점。결론：사구당뇨병방치관리가시행전과단대복무，고정의환관계，체도진료개체화，촉진취진，치료여전진적의종성，유효공제환자혈당。
Objective: To investigate the attainment of the indicators of glycemic control of the patients with diabetes and analyze the general practice service. Methods:The investigation used the face to face questionnaires at home, and the results were analyzed with logistic and other methods. Results: Blood pressure, medication, diabetes cognition, and glycosylated hemoglobin were the main factors which impacted the comprehensive attainment of indicators of the community management of glycemic control. Conclusion: Community prevention, treatment and management of diabetes can try to use the general practitioners team service that ifxes the relationship between the doctors and patients, achieves the individualized treatment, promotes the compliance of visits, treatment and referral so as to effectively control of blood glucose of the patients.
