目的:探讨人类白细胞抗原Ⅱ类(HLAⅡ类)基因多态性与中国多发性硬化(MS)患者遗传易感性的关系。方法通过OVID、Embase、中国期刊全文数据库(CNKI)和万方数据库等,检索国内外发表的关于HLAⅡ类基因与中国人群MS的病例对照文献,应用RevMan 5.2软件进行Meta分析。结果共纳入15篇相关文献,各研究HLA-DQB1*0601、DRB1*0405、DRB1*1501、DRB1*1602、DPB1*1301基因结果未见明显发表偏倚。Meta分析结果显示:(1)与对照组相比, MS的HLA-DRB1*1602基因表达较高[OR=2.35,95%CI(1.43,3.86),P=0.0007];(2)与对照组相比,MS 的 HLA-DQB1*0601、DRB1*0405、DRB1*1501、DPB1*1301的基因表达无统计学意义。结论现有有限证据表明,HLA-DRB1*1602基因是MS的易患基因,但其余HLAⅡ类基因与MS的关系尚不确定,需要进行更多大样本、设计严谨的高质量研究来进一步证实。

목적:탐토인류백세포항원Ⅱ류(HLAⅡ류)기인다태성여중국다발성경화(MS)환자유전역감성적관계。방법통과OVID、Embase、중국기간전문수거고(CNKI)화만방수거고등,검색국내외발표적관우HLAⅡ류기인여중국인군MS적병례대조문헌,응용RevMan 5.2연건진행Meta분석。결과공납입15편상관문헌,각연구HLA-DQB1*0601、DRB1*0405、DRB1*1501、DRB1*1602、DPB1*1301기인결과미견명현발표편의。Meta분석결과현시:(1)여대조조상비, MS적HLA-DRB1*1602기인표체교고[OR=2.35,95%CI(1.43,3.86),P=0.0007];(2)여대조조상비,MS 적 HLA-DQB1*0601、DRB1*0405、DRB1*1501、DPB1*1301적기인표체무통계학의의。결론현유유한증거표명,HLA-DRB1*1602기인시MS적역환기인,단기여HLAⅡ류기인여MS적관계상불학정,수요진행경다대양본、설계엄근적고질량연구래진일보증실。
Objective To investigate the association of the polymorphism of the HLAⅡ gene and multiple sclerosis (MS) in Chinese population. Methods A computer-based online search was conducted in OVID, Embase Database, China National Knowledge Infrastructure, and Wanfang Database. Case-control studies addressing the association of the polymorphism of the HLAⅡgene and multiple sclerosis in Chinese population were included in this review. The Meta-analysis software, RevMan 5.2 was applied for investigating heterogeneity, pooled OR value and 95% CI among individual studies. Results Fifteen case-control studies were included after screening and application of inclusion and exclusion criteria. META analysis showed:(1)Compared with control group, MS patients HLA-DRBI*1602 gene expression was rather high[OR=2.35, 95%CI(1.43, 3.86), P=0.000 7];(2) Compared with control group, the HLA DQB1*0601, DRB10405, DRB1*1501, DPB1*1301 of MS gene expression showed no value to statistics. Conclusion The limited evidence at present show that HLA-DRB1*1602 gene is liability, but the relationship between HLAⅡ gene and MS is still couldn't be confirmed. It should be further confirmed through more samples, more rigorously designed and high quality investigation.
