目的:探讨经皮球囊扩张椎体后凸成形术治疗椎体压缩性骨折的临床效果。方法应用经皮球囊扩张椎体后凸成形术治疗骨质疏松性椎体骨折的82例为研究对象。对所有患者分别进行术前、术后3 d、3个月和12个月视觉疼痛模拟评分(VAS)、测量标准侧位X线片上病椎高度、椎体后凸畸形角(Cobb's角)。门诊随访12~20个月,平均15.6个月。比较手术前后的临床疗效是否存在差异。结果82例患者经皮球囊扩张椎体后凸成形术治疗,VAS评分由术前(8.07±0.21)分降为术后3 d(2.81±0.41)分(P<0.01),12个月随访为(2.61±0.25)分(P<0.01)。椎体高度由术前为(1.65±0.16)cm恢复至术后3 d(2.08±0.18)cm(P<0.05),随访12个月为(1.96±0.13)cm(P<0.05)。Cobb's角由术前29.56°±4.16°恢复至术后3 d的14.42°±3.83°(P<0.01),12个月后随访为16.154°±4.315°(P<0.01)。结论经皮球囊扩张椎体后凸成形术可有效缓解骨质疏松性椎体骨折引发的疼痛,重建病椎外形,是一种治疗椎体压缩骨折有效方法。

목적:탐토경피구낭확장추체후철성형술치료추체압축성골절적림상효과。방법응용경피구낭확장추체후철성형술치료골질소송성추체골절적82례위연구대상。대소유환자분별진행술전、술후3 d、3개월화12개월시각동통모의평분(VAS)、측량표준측위X선편상병추고도、추체후철기형각(Cobb's각)。문진수방12~20개월,평균15.6개월。비교수술전후적림상료효시부존재차이。결과82례환자경피구낭확장추체후철성형술치료,VAS평분유술전(8.07±0.21)분강위술후3 d(2.81±0.41)분(P<0.01),12개월수방위(2.61±0.25)분(P<0.01)。추체고도유술전위(1.65±0.16)cm회복지술후3 d(2.08±0.18)cm(P<0.05),수방12개월위(1.96±0.13)cm(P<0.05)。Cobb's각유술전29.56°±4.16°회복지술후3 d적14.42°±3.83°(P<0.01),12개월후수방위16.154°±4.315°(P<0.01)。결론경피구낭확장추체후철성형술가유효완해골질소송성추체골절인발적동통,중건병추외형,시일충치료추체압축골절유효방법。
Objective To evaluate the Clinical effect of percutaneous kyphoplasty for vertebral compression fractures. Methods Eighty-two patients undergoing percutaneous kyphoplasty were retrospectively analysed.The evaluation was based on the visual analogue scales(VAS) and vertebral height and Cobb's angle recorded at 3 days, 3 months and 12 moths postoperatively. The duration of follow up after operation were 12 to 20 months with a mean of 15.6 months. Results The VAS score reduced from 8.07±0.21 before operation to 2.81±0.41 (P<0.01) and vertebral height increased from(1.65±0.16)cm to(2.08±0.18)cm (P<0.05), while Cobb's angle decreased from 29.56°±4.16°to 14.42°±3.83° (P<0.01) after operation 3 days.There was no significant VAS score, vertebral height and Cobb's angle changed at 12-month post-operative. Conclusion Percutaneous kyphoplasty can relieve pain, improve physical function, and restore the height of the collapsed vertebrae.
