目的：探讨人类白细胞抗原（HLA）-DQB1等位基因多态性与新生儿溶血病（HDN）之间的关系。方法对117例HDN患儿和112例非新生儿溶血病（NHDN）标本，采用PCR-SSO方法进行HLA-DQB1基因分型，对基因频率和疾病发生的相对危险度（RR）进行研究。结果 HDN患儿中HLA-DQB1*0301的基因频数较NHDN 显著升高（P<0.05），其RR值为1.721；含有HLA-DQB1*0301的HDN患儿的总胆红素值较不含该基因的患儿高，其差异有显著性（P=0）。结论HLA-DQB1*0301与HDN的易感性呈正相关，可能是其易感基因。
목적：탐토인류백세포항원（HLA）-DQB1등위기인다태성여신생인용혈병（HDN）지간적관계。방법대117례HDN환인화112례비신생인용혈병（NHDN）표본，채용PCR-SSO방법진행HLA-DQB1기인분형，대기인빈솔화질병발생적상대위험도（RR）진행연구。결과 HDN환인중HLA-DQB1*0301적기인빈수교NHDN 현저승고（P<0.05），기RR치위1.721；함유HLA-DQB1*0301적HDN환인적총담홍소치교불함해기인적환인고，기차이유현저성（P=0）。결론HLA-DQB1*0301여HDN적역감성정정상관，가능시기역감기인。
Objective To explore the relationship between HLA-DQB1 alleles polymorphism and hemolytic disease newborn(HDN). Methods 117 HDN children and 112 Non-hemolytic disease newborn(NHDN) children used polymerase chain reaction sequence specific oligonucleotide (PCR-SSO) method. The results of the two groups were analyzed by case control study to discover the different distribution in gene,haplotype and to investigate the relative risk (RR) of HDN. Results The gene frequencies of HLA-DQB1*0301 in HDN group were significantly higher (P<0.05) than that in NHDN group, and the RR was 1.721. The total bilirubin of the HDN children whose gene contained HLA-DQB1*0301 was higher than the children whose gene did not contain the gene. There were significant difference between two groups (P=0). Conclusion A positive correlation is found between HLA genetic polymorphism and the susceptibility of HDN. HLA-DQB1*0301 may be the susceptibility gene for HDN.
