目的 使用OQAS视觉质量分析系统比较超声乳化白内障摘除术后联合植入亲水性或疏水性丙烯酸酯人工晶状体(IOL)在4 mm瞳孔下眼内散射光和视觉质量的差异,并探讨眼内散射光与视觉质量的关系.方法 回顾性病例系列研究.收集2012年3~12月在天津市眼科医院就诊的年龄相关性白内障术后植入亲水性或疏水性丙烯酸IOL至少3个月的患者56例(68只眼).按照IOL的材质分为3组:分别为1组:一体肝素表面处理亲水性丙烯酸人工晶状体(HQ201HEP) 28只眼;2组:三体疏水丙烯酸酯Tecnis 9003 (ZA9003) 20只眼;3组:三体疏水性丙烯酸酯Sensar (AR40E) 20只眼.使用OQAS视觉质量分析系统定量测量在4 mm瞳孔下不同材质IOL的客观散射指数(OSI)和视觉质量:调制传递函数截止频率(MTF Cut off)、斯特尔比率(Strehl ratio,SR),并进行比较.结果 4mm瞳孔下HQ201HEP、ZA9003和AR40E的眼内OSI、MTF Cut off、SR之间的差异有统计学意义(F值分别为10.016、6.017、6.464,P值分别为0.000、0.004、0.003);两两比较:HQ201HEP组和ZA9003组,HQ201HEP组和AR40E组之间的OSI、MTF Cut off、SR的差异有统计学意义(P<0.05).HQ201HEP组中OSI与MTF Cut off、SR及患者的年龄之间有相关性差异有统计学意义(P<0.05);ZA9003组中OSI与MTF Cut off、SR之间有相关性差异有统计学意义(P <0.05); AR40E组OSI与MTF Cut off、SR之间有相关性差异有统计学意义(P<0.05).结论 疏水性丙烯酸酯IOL的眼内散射值较亲水性丙烯酸酯IOL的眼内散射值小,并且视觉质量较好;散射光严重影响人工晶状体眼的视觉质量.

Objective To compare the Objective Scatter Index and optical performance of intraocular lens using a double-pass method.Methods A retrospective case series study.Three different kinds of acrylic intraocular lenses were implanted into 68 eyes in cataract surgery,including hydrophilic acrylic intraocular lens HQ201HEP 28 eyes; hydrophobic acrylic intraocular lens ZA9003 20 eyes; hydrophobic acrylic intraocular lens (IOL) AR40E IOL 20eyes.Straylight on the retina and optical quality (MTF Cut off,SR) of the eyes at 4rm pupil size was measured with double-pass system.Difference among groups was evaluated by analysis of variance (ANOVA).Relationship between straylight parameters (OSI) and optical quality was evaluated by Pearson correlation analysis.Results At 4 mm pupil size,there was a significant difference between the objective scatter index (OSI),MTF Cut off,SR values of the HQ201HEP,ZA9003 and AR40E group (P <0.05).Pairwise comparisons,there was a significant differences between the HQ201HEP and ZA9003 group,the HQ201HEP and AR40E group (P <0.05).In HQ201HEP group,ZA9003 group,AR40E group,there was a significant correlation between the OSI and MTF Cut off,SR (P <0.05).Conclusions At 4 mm pupil size,the straylight in hydrophilic acrylic IOLs is higher than in hydrophobic acrylic IOLs,hydrophobic acrylic IOLs shows better visual quality than hydrophilic acrylic IOLs.There is a significant negative correlation between the straylight parameters (OSI) and optical luality.
