目的 比较2型糖尿病患者与年龄与性别匹配正常人的血清癌抗原(CA)199水平.方法 选择60例2型糖尿病患者(糖尿病组)和40例健康体检者(对照组),检测并比较两组血清CA199、空腹血糖、餐后血糖及糖化血红蛋白水平.结果 糖尿病组血清CA199水平为(45.2±18.6) U/ml,高于对照组的(11.3±6.4) U/ml,差异有统计学意义(P<0.01),Pearson相关分析显示血清CA199水平与空腹血糖、餐后血糖及糖化血红蛋白水平无相关性.结论 CA199是诊断胰腺癌的指标,但也可以作为糖尿病引起的胰腺组织损伤的一个标志.检测2型糖尿病患者CA199水平应该高于其正常参考值范围去评价,以利于正确评估良恶性胰腺疾病.2型糖尿病患者轻度升高的CA199或许反映了患者胰腺外分泌功能紊乱.

Objective To investigate serum CA 199 levels in type 2 diabetic patients in comparison with age-and gender-matched control subjects.Methods We recorded FPG,2hPG,HbA1c levels and serum CA 199 levels in 60 type 2 diabetic patients and 40 controls.Abdominal CT was performed in order to eliminate abdominal malignancy in the diabetic groups.Results The average CA 199 level was 45.2 ±18.6U/mL for diabetic patients whereas it was 11.3 ± 6.4 U/mL for the control group (P < 0.01).Serum CA 199 had no correlation with FPG,2hPG and HbA1c levels.For all patients,abdominal CT showed no pancreatic abnormalities.Conclusion CA199 is used in the diagnosis of pancreatic cancer but is also a marker of pancreatic tissue damage that might be caused by diabetes.We propose that a higher cut-off value of CA 199should be used in diabetics to differentiate benign and malignant pancreatic disease,and subtle elevations of CA 199 in diabetics should be considered as the indication of exocrine pancreatic dysfunction.
