目的 探索激光治疗早产儿视网膜病变(ROP)疗效及预后的影响因素.方法 回顾性分析2011年3月至2012年7月在吉林大学第一医院新生儿科收治的20例行810 nm激光治疗ROP早产儿的相关临床数据,并进行统计学分析.结果 共有20例患儿(40只眼)行激光治疗.Ⅰ区病变18只眼,Ⅱ区病变22只眼.平均出生体重1 433.25 g,平均胎龄30.05周,治疗时矫正胎龄31~43周,平均矫正胎龄37.25周.一次激光治愈38只眼,治愈率为95% (38/40).病变部位与统计的各临床因素(孕次、产次、胎龄、出生体重、矫正胎龄、激光治疗年龄、新生儿低血糖、坏死性小肠结肠炎、肺炎、败血症、新生儿窒息等11个临床因素)差异无统计学意义(P>0.05).结论 810 nm激光光凝治疗ROP是安全有效的,治疗效果与患儿临床因素关系不大,与治疗时机的选择有关.

Objective To explore the treatment outcome for retinopathy of prematurity by 810 nm diode laser photocoagulation and the prognostic factors.Methods The clinic data from 20 ROP preterm infants treated in the first affiliated hospital of Jilin University from March 2011 to July 2012 were retrospectively analyzed.Results A total of 20 patients(40 eyes)underwent laser therapy.There were 18 eyes with zone 1,22 eyes with zone 2.The mean birth weight and the gestational age was 1433.25g and 30.05 weeks,respectively.The corrected gestational age at the time of operation was 31 weeks to 43 weeks,the mean corrected gestational age was 37.25 weeks.There were no significant diffreneces in gravidity,parity,gestational age,birth weight,corrected gestational age,age at laser,neonatal hypoglycemia between zone 1 and zone 2 (P >0.05).Conclusion 810 nm diode laser retinal photocoagulation is a safe and effective procedure for treatment of ROP.The treatment outcome is associated with the diode laser opportunity rather than such certain clinical risk factors.
