目的观察常规疗法加乙酰半胱氨酸口服对慢性阻塞性肺疾病(COPD)患者肺通气功能及血清细胞因子IL-8、TNF-a、IL-10改变的影响。方法将COPD急性加重期的85例患者随机分为A、B两组。A组(治疗组)45例,在常规治疗基础上加用乙酰半胱氨酸胶囊400 mg口服,每日3次,连用10 d。B组(对照组)40例,仅常规治疗,不加用乙酰半胱氨酸胶囊口服。治疗前后观察肺通气功能、血气分析及血清细胞因子(IL-8、TNF-a、IL-10)的改变。结果 A、B两组患者经治疗后肺通气功能及动脉血气分析指标均有好转,血清IL-8、TNF-a、IL-10的表达亦均有改善,并且A组优于B组(P<0.05)。结论乙酰半胱氨酸可有效改善COPD患者呼吸功能,改善血清IL-8、TNF-a、IL-10的表达。

목적관찰상규요법가을선반광안산구복대만성조새성폐질병(COPD)환자폐통기공능급혈청세포인자IL-8、TNF-a、IL-10개변적영향。방법장COPD급성가중기적85례환자수궤분위A、B량조。A조(치료조)45례,재상규치료기출상가용을선반광안산효낭400 mg구복,매일3차,련용10 d。B조(대조조)40례,부상규치료,불가용을선반광안산효낭구복。치료전후관찰폐통기공능、혈기분석급혈청세포인자(IL-8、TNF-a、IL-10)적개변。결과 A、B량조환자경치료후폐통기공능급동맥혈기분석지표균유호전,혈청IL-8、TNF-a、IL-10적표체역균유개선,병차A조우우B조(P<0.05)。결론을선반광안산가유효개선COPD환자호흡공능,개선혈청IL-8、TNF-a、IL-10적표체。
Objective To observe changes and effects of conventional treatment plus oral Acetylcysteine on COPD patients' pulmonary ventilation function and serum cytokines IL-8, TNF-a, and IL-10. Methods Eighty-five patients with acute exacerbation of COPD were randomly divided into Group A and Group B. Forty-five patients in group A which were treated with acetylcysteine, besides conventional treatments, also were taken acetylcysteine pills (400 mg) tid for 10 days. At the same time, forty patients in group B for comparison only were received conventional treatment without any acetylcysteine pills. The changes of lung function, blood gas analysis and serum cytokines IL-8, TNF-a, and IL-10 before and after the treatment were observed. Results Pulmonary ventilation function, arterial blood gas and serum cytokines IL-8, TNF-a and IL-10 were improved in all patients from both groups A and B. And group A is even better than group B (P<0.05). Conclusion Acetylcysteine can effectively improve COPD patients' respiratory function and indicators of serum IL-8, TNF-a and IL10.
