目的探讨采用弹性复位固定治疗长节段骨干粉碎性骨折的临床技巧和疗效。方法选取2005年1月~2011年5月期间在我院治疗的长节段骨干粉碎性骨折患者49例为观察组,采用弹性复位固定方案进行治疗,同时选取这一时期采用髓内取骨植骨内固定治疗的50例长节段骨干粉碎性骨折患者为对照组,并对所有患者进行为期6~18个月的随访。结果本组49例均痊愈出院,经X线复查,本组患者皆无成角,对线好,骨折端无一例出现移位现象,评定结果中效果明显者29例,有效20例,有效率为100%,经随访可知观察组患者均未出现功能障碍、肢体畸形或者钢板出现断裂或者松动等现象;对照组患者效果明显25例,有效19例,效果差6例,总有效率为88%;两组患者总有效率差异有统计学意义(P<0.05)。结论采用弹性复位固定治疗长节段骨干粉碎性骨折固定可靠,创伤小,并发症少,无需特殊器械,骨折愈合时间短,操作简单,消耗时间少,是科学、有效的治疗方法,值得临床推广应用。

Objective To explore the skill and clinical effect of elastic replacement and fixation on long-segment diaphysis comminuted fracture. Methods 49 cases with long-segment diaphysis comminuted fracture collected in our hospital from January 2005 to May 2011 were chosen as the observation group and the program of elastic replacement and fixation was measured. 50 cases with long-segment diaphysis comminuted fracture collected during the same period were treated as the control group with internal fixation with medullary bone grafting. All the patients were follow-up visited in the coming 6-18 months. Results 49 cases were recurred and no angel and no displacement in the fracture appeared. The results showed that 29 cases were excellent, 20 cases were effective and the effective rate was 100%. And the follow-up visit showed that no dysfunction, limbs deformity or steel rupture/start appeared in the observation group. In the control group, the results showed that 25 cases were excellent, 19 cases were effective, 6 cases were bad and the effective rate was 88%. There was significant difference in the effective rate between two groups(P < 0.05). Conclusion Elastic replacement and fixation on long-segment diaphysis comminuted fracture has reliable fixation, less damage, less complications and no special instrument, shorter fracture healing time, simple operation, less consumption time, and is scientific and effective treatment method, and worthy of clinical application.
