目的探讨自行研制U形泪道吻合支撑管在泪道断裂修复术中的临床应用及效果.方法回顾性研究于2010年1月至2011年6月在抚顺市第三医院眼科应用U形泪道吻合支撑管,在显微镜下寻找到泪管两断端后经上下泪点直接插入远、近两侧断端泪管,再经泪囊插入鼻泪管至鼻腔,形成U形支撑治疗泪管断裂伤患者40例的临床资料.结果 40例患者,均一期愈合,无感染,术中插管顺利,手术时间仅是传统手术时间1/2,痛苦少.拔管后随访3～12个月,泪点均不同程度轻微扩大.其中2例下泪点及部分泪管撕裂,但受泪情况良好,1例术后仍明显溢泪,39例效果良好,成功率为97.5％ (39/40).结论自行研制U形泪道吻合支撑管应用于泪道断裂伤修复术,效果良好,不仅缩短手术时间,减轻痛苦,提高手术成功率,而且术后置管稳定性佳,外观良好.

Objective To investigate the clinical application and effect of self-developed U-supporting tube of lacrimal duct anastomosis.Methods A retrospective study.Clinical data of 40 cases of trauma patients with nasolacrimal duct fracture were reviewed.Application of the U lacrimal ductanastomotic support tube under a microscope to find nasolacrimal duct ends after the upper and lower lacrimal point directly into the far and near broken side by nasolacrimal duct were used,and then lacrimal sac into the nasolacrimal duct to the nasal cavity,the formation of U type support was used for treatment.Result The surgery went well,intubation was smoothly and all 40 patients healed without infection.Operation time was only 1/2 traditional operation time,pain was reduced.During the follow-up of 3-12 months after extubation,tears were slightly expanded,including two cases of lacrimal points and part of the tear ducts tear,but in good condition by tears,1 case still had obvious epiphora,39 cases were satisfactory.The success rate was 97.5％ (1/40).Conclusions The self-developed U-supporting tube used in the lacrimal duct rupture surgery,and the result is good,not only shortens the operation time,relieves pain,and improves the success rate.After surgery,stability of supporting tube is good,and there is good appearance.
