目的 评价三种不同麻醉方法对腹腔镜下胆囊切除术中应激反应及血气变化的影响,对比各种方法的疗效.方法 选择2010年1月至2012年10月于我院就诊行腹腔镜下胆囊切除术的150例患者的临床资料进行回顾性分析,随机抽取全麻组56例、硬膜外麻醉组52例,全麻复合硬膜外麻醉42例,观察麻醉前后、气腹后10 min时心率、平均动脉压、C反应蛋白、PETCO2、SpO2各项指标,并运用分析各关指标.结果 全麻复合硬膜外麻醉患者在术中心率、平均动脉压、C反应蛋白等各项应激反应指标及PETCO2、SpO2指标较单纯全麻及单纯硬膜外麻醉改变比较,差异有统计学意义(P＜0.05).结论 全麻复合硬膜外麻醉下,患者的术中应激反应较小,生化指标改变波动较小.

목적 평개삼충불동마취방법대복강경하담낭절제술중응격반응급혈기변화적영향,대비각충방법적료효.방법 선택2010년1월지2012년10월우아원취진행복강경하담낭절제술적150례환자적림상자료진행회고성분석,수궤추취전마조56례、경막외마취조52례,전마복합경막외마취42례,관찰마취전후、기복후10 min시심솔、평균동맥압、C반응단백、PETCO2、SpO2각항지표,병운용분석각관지표.결과 전마복합경막외마취환자재술중심솔、평균동맥압、C반응단백등각항응격반응지표급PETCO2、SpO2지표교단순전마급단순경막외마취개변비교,차이유통계학의의(P＜0.05).결론 전마복합경막외마취하,환자적술중응격반응교소,생화지표개변파동교소.
Objective To explore the impacts of three different anesthetic methods on stress response and blood gas changes during laparoscopic cholecystectomy.Methods The clinical data on 150 patients who had undergone laparoscopic cholecystectomy in our hospital during the period of January 2010 to December 2012 were retrospectively analyzed.56 patients were in the group of general anesthesia,52 were in the group of epidural anesthesia,and 42 were in the group of general anesthesia combined with epidural anesthesia.Heart rate,mean arterial pressure,C-reactive protein,PETCO2,and SpO2 were observed before and after anesthesia and 10min after formation of pneumoperitoneum.Results Heart rate,mean arterial pressure,C-reactive protein,PETCO2,and SpO2 differed significantly between the group of general anesthesia combined with epidural anesthesia and the group of general anesthesia or epidural anesthesia alone (P＜0.05).Conclusions Under general anesthesia combined with epidural anesthesia,patients develop less stress response and have less fluctuation in biochemical indicators.
