Type 2 diabetes mellitus in elderly patients%Tumor necrosis factor- α%SNP
目的:研究TNF-α基因多态性在浙江台州地区2型糖尿病患者中的分布与相关性。方法利用DNA测序法对100例老年2型糖尿病患者(观察组)及50例健康体检者(对照组)的TNF-α基因-308位点和-238位点进行基因检测与分析。结果观察组FPG、TNF-α、TG、TC、胰岛素抵抗指数(IRI)分别为(10.68±2.75)mmol/L、(2.01±0.25)ug/L、(2.43±1.15)mmol/L、(5.49±1.63)mmol/L及4.65±0.97,均高于对照组,差异均有统计学意义(均P<0.05),HDL为(1.23±0.28)mmol/L,低于对照组,差异有统计学意义(P<0.05)。而BMI、LDL[(25.34±2.41)kg/m2、(3.26±1.3)mmol/L]与对照组比较,差异无统计学意义(P>0.05)。TNF-α基因-308位点在观察组和对照组GA+AA基因频率分别为31%和10%,A等位基因频率分别为16%和5%,差异有统计学意义(P<0.05)。而-238位点未见突变,两组间比较无明显差异。结论 TNF-α基因G-308A单核苷酸多态性与2型糖尿病存在相关性。

Objective To investigate the polymorphism of TNF- αgene in patients with type 2 diabetes mellitus (T2DM) in Taizhou region of Zhejiang province. Methods Blood specimens were col ected from 100 elderly patients with type 2 diabetes mel itus and 50 normal elderly subjects from Taizhou region of Zhejiang Province. Genomic DNA was extracted from blood sam-ples. The primers of PCR for the SNPs of G308A and G238A were optimized and the polymorphism of amplicons was analyzed by DNA sequencing. Results FPG, TNF- α, TG, TC and insulin resistance index in T2DM group were 10.68±2.75mmol/L, 2.01± 0.25μg/L, 2.43±1.15mmol/L, 5.49±1.63mmol/L and 4.65±0.97, respectively, which were al higher than those in control group (P<0.05). The HDL- C(1.23±0.28mmol/L) was lower than that in controls(P<0.05), while there was no difference in BMI(25.34± 2.41kg/m2) and LDL(3.26±1.3mmol/L) between T2DM patients and controls (P>0.05).The frequency of GA+AA genotype in TNF- α- 308 gene of T2DM group and control group was 31%and 10%,and A al ele 16%and 5%, respectively (P<0.05);while there was no difference in genotype and A al ele frequency in TNF- a- 238 gene between two groups. Conclusion The 308 G- A polymorphism of TNF- αpromoter gene may be associated with type 2 diabetes mel itus in elderly patients from Taizhou region of Zhejiang province.
