目的:探讨血清降钙素原(PCT)及急性生理与慢性健康评分Ⅱ(APACHEⅡ)监测对感染性休克患者病情及预后的评估价值。方法选取本院收治的83例感染性休克患者作为观察组,选择90例健康体检者作为对照组,对比两组血清PCT含量与APACHE Ⅱ评分,并对观察组的复苏液体量、PCT及APACHE Ⅱ等的相关性,进行直线回归分析。结果观察组患者复苏前后的PCT和APACHE Ⅱ均显著高于对照组(PCT:t=26.541、21.538,P<0.05;APACHEⅡ:t=25.114、6.959,P <0.05),且观察组复苏后的PCT和APACHEⅡ与复苏前相比,显著下降(PCT:t=12.959,P<0.05;APACHEⅡ:t=11.633,P<0.05);复苏前后PCT与APACHEⅡ均呈显著正相关(复苏前:r=0.804,P=0.011;复苏后:r=0.681, P=0.031),而复苏液体量与PCT呈显著负相关(r=-0.631,P<0.05),而与APACHEⅡ无相关性(r=0.157,P>0.05)。结论 PCT的变化与感染性休克患者的病情有关,对患者的APACHEⅡ评分与血清PCT含量进行监测,有利于对其病情及预后进行准确地评估,值得临床上推广使用。

목적:탐토혈청강개소원(PCT)급급성생리여만성건강평분Ⅱ(APACHEⅡ)감측대감염성휴극환자병정급예후적평고개치。방법선취본원수치적83례감염성휴극환자작위관찰조,선택90례건강체검자작위대조조,대비량조혈청PCT함량여APACHE Ⅱ평분,병대관찰조적복소액체량、PCT급APACHE Ⅱ등적상관성,진행직선회귀분석。결과관찰조환자복소전후적PCT화APACHE Ⅱ균현저고우대조조(PCT:t=26.541、21.538,P<0.05;APACHEⅡ:t=25.114、6.959,P <0.05),차관찰조복소후적PCT화APACHEⅡ여복소전상비,현저하강(PCT:t=12.959,P<0.05;APACHEⅡ:t=11.633,P<0.05);복소전후PCT여APACHEⅡ균정현저정상관(복소전:r=0.804,P=0.011;복소후:r=0.681, P=0.031),이복소액체량여PCT정현저부상관(r=-0.631,P<0.05),이여APACHEⅡ무상관성(r=0.157,P>0.05)。결론 PCT적변화여감염성휴극환자적병정유관,대환자적APACHEⅡ평분여혈청PCT함량진행감측,유리우대기병정급예후진행준학지평고,치득림상상추엄사용。
Objective To investigate the evaluating value of procalcitonin (PCT) and physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) monitoring of condition and prognosis in patients with septic stock. Methods Total of 83 patients with septic stock were divided into observation group, and 90 cases of healthy physical examination were divided into control group. Serum PCT levels and APACHEⅡscores of two groups were compared, respectively. And correlation of recovery liquid, PCT and APACHEⅡscores of observation group were analyzed by linear regression. Results Before and after recovery, the levels of serum PCT and APACHEⅡscores of patients in observation group were signiifcantly higher than that of control group (PCT:t=26.541 and 21.538, P<0.05;APACHEⅡ:t=25.114 and 6.959, P<0.05), and after recovery, those of observation group were signiifcantly lower than before (PCT:t=12.959, P<0.05;APACHEⅡ:t=11.633, P<0.05). PCT and APACHEⅡscores of observation group were signiifcantly positive correlation, before and after recovery (before recovery:r=0.804, P=0.011;after recovery:r=0.681, P=0.03), but recovery liquid had signiifcant negative correlation with PCT (r=-0.631, P<0.05), while there was no correlation (r=0.157, P>0.05) with APACHEⅡ. Conclusions Change of PCT is associated with the condition of patients with septic shock. Providing PCT and APACHEⅡmonitoring for patients is beneifcial to accurately assess the condition and prognosis, and worth for clinical use.
