目的检测结直肠癌患者( Dukes B期、C期)术中植入氟尿嘧啶植入剂后,手术前后外周血Survivin、CD44V6的变化,探讨其对肿瘤微转移及预后的影响。方法64例结直肠癌患者随机均分为两组:治疗组行标准根治术,术中植入氟尿嘧啶植入剂;对照组予标准根治术而术中未应用此药。采用RT-PCR法测定手术前后外周血中Survivin表达水平,流式细胞术检测CD44V6含量。结果64例结直肠癌患者(Dukes B期、C期)术前各组 Survivin、CD44V6表达的差异无统计学意义,术后治疗组和对照组Survivin、CD44V6水平明显低于术前(P<0.05),且治疗组Survivin、CD44V6水平明显低于对照组( P<0.05)。结论术中植入氟尿嘧啶植入剂可通过降低Survivin、CD44V6的表达来抑制肿瘤的微转移,对结直肠癌的预后有着重要意义,有望提高结直肠癌术后的远期疗效。

목적검측결직장암환자( Dukes B기、C기)술중식입불뇨밀정식입제후,수술전후외주혈Survivin、CD44V6적변화,탐토기대종류미전이급예후적영향。방법64례결직장암환자수궤균분위량조:치료조행표준근치술,술중식입불뇨밀정식입제;대조조여표준근치술이술중미응용차약。채용RT-PCR법측정수술전후외주혈중Survivin표체수평,류식세포술검측CD44V6함량。결과64례결직장암환자(Dukes B기、C기)술전각조 Survivin、CD44V6표체적차이무통계학의의,술후치료조화대조조Survivin、CD44V6수평명현저우술전(P<0.05),차치료조Survivin、CD44V6수평명현저우대조조( P<0.05)。결론술중식입불뇨밀정식입제가통과강저Survivin、CD44V6적표체래억제종류적미전이,대결직장암적예후유착중요의의,유망제고결직장암술후적원기료효。
Objective To observe the pre-operational and post-operational changes of the expressions of Survivin, CD44V6 in patients with colorectal cancer after intra-abdominal implantation fluorouracil sustained release implant. Methods The peripheral blood levels of Survivin were detected by RT-PCR and CD44 V6 by FCM in 64 patients ( fluorouracil sustained release implant was implanted intra-abdominally in 32 patients undergoing radical colorectal cancer surgery in the experimental group,while 32 cases undering radical surgery only served as controls) diagnosed with colon cancer. Results No significant differences of Survivin and CD44V6 were detected between the two groups before operation(P>0.05). Survivin and CD44V6 levels were lower (P<0.05)in the patients of the ex-perimental group than the controls after operation. Conclusion Fluorouracil sustained release implant can inhibit the expression of Survivin and CD44V6 to effect the colorectal cancer micro transfer.
