目的探讨预防性补充氨基酸螯合铁剂对34周以上早产儿血红蛋白的影响。方法将75例34~37周早产儿随机分为三组。对照组不预防服用药物;低铁组生后2周至6个月口服甘氨酸螯合铁[铁元素1mg/(kg·d)];高铁组生后2周至6个月口服甘氨酸螯合铁[铁元素2mg/(kg·d)]。观察血红蛋白变化情况及服药后反应。结果低铁组、高铁组贫血发生率与对照组比较差异均有显著性;低铁组、高铁组血红蛋白分别与对照组比较,自生后4个月差异均有显著性。结论34~37周早产儿生后2周至6个月服用甘氨酸螯合铁可预防出现贫血,但剂量增加未提高预防效果。

목적탐토예방성보충안기산오합철제대34주이상조산인혈홍단백적영향。방법장75례34~37주조산인수궤분위삼조。대조조불예방복용약물;저철조생후2주지6개월구복감안산오합철[철원소1mg/(kg·d)];고철조생후2주지6개월구복감안산오합철[철원소2mg/(kg·d)]。관찰혈홍단백변화정황급복약후반응。결과저철조、고철조빈혈발생솔여대조조비교차이균유현저성;저철조、고철조혈홍단백분별여대조조비교,자생후4개월차이균유현저성。결론34~37주조산인생후2주지6개월복용감안산오합철가예방출현빈혈,단제량증가미제고예방효과。
Objective To explore the effect of amino acid chelated iron on hemoglobin level in premature infant whose gestational age was over 34 weeks. Method There were 75 preterm infants employed in our study who were ad-mitted to two hospitals from April 2010 through April 2011 for gestational age during 34 to 37 weeks. They were ran-domized divided into three groups. Group one(n=35): no amino acid chelated iron was given,in other two groups ( n=20 for each group) ,infants were given amino acid chelated iron from 2 weeks old to six months,with iron ele-ment 1mg/(kg·d),or 2mg/(kg·d) respectively. Hemoglobin level in blood had been recorded during the six months. Result 17 cases(48. 57%) had anemia symptom at four months in control group,4 cases in experimental group one and 1 case in experimental group two at five months. There was significance different hemoglobin level be-tween control group and experimental groups during 6 months(P<0. 05). All cases in experimental group hadn't got significant side effect. The different of hemoglobin between control group and experimental group had statistical sig-nificance after four months. Conclusion Premature infant(gestational age 34 to 37 weeks) would develop anemia symptom after four months in case of not taking iron after birth. Amino acid chelated iron is capable of prevention a-nemia in premature infant without significant side effect. The dosage of 2mg/(kg·d) could not get better prevention than dosage of 1mg/(kg·d).
