目的:研究肥胖与原发性骨质疏松症的关联性。方法基于社区多级整群抽样的方法,获得2498例18岁以上居民的相关信息,通过HOLOGIC双能X线骨密度仪测定骨密度,通过分层及多因素logistic回归分析,对肥胖与原发性骨质疏松症的关联性进行研究。结果按照性别分层,男性人群中结果显示肥胖和原发性骨质疏松症呈负关联(χMH2=26.845,P<0.001),同时也可以看到Ⅱ度肥胖者的原发性骨质疏松症患病率要高于Ⅰ度肥胖者(11.1%>6.6%);女性人群中肥胖和原发性骨质疏松症的关联无统计学意义(χMH2=2.007,P=0.157);如不分层,资料显示人群肥胖和原发性骨质疏松症呈负关联(χMH2=7.016,P=0.008)。按照年龄分层后,各年龄段(除age≥70组)均显示肥胖和原发性骨质疏松症呈负关联(χMH 2值依次为4.152、15.306、10.212、18.499、3.341,P值依次为0.042、<0.001、0.001、<0.001、0.068)。通过多因素logistic回归分析,调整性别、年龄后,结果显示肥胖和原发性骨质疏松症呈负关联(OR=0.703,OR的95%C.I.为0.620~0.799)。结论骨质疏松症与肥胖有密切联系,体重管理在预防骨质疏松方面的作用是个公共健康问题,值得广泛关注。

Objective To study the correlation between obesity and primary osteoporosis.Methods Adopting the multi-stage cluster sampling method based on community, the information of 2498 residents elder than 18 years old was obtained.The bone mineral density ( BMD) was measured using the HOLOGIC dual energy X-ray absorptiometry.The results were analyzed using stratification analysis and the multivariate logistic regression analysis, in order to investigate the correlation between obesity and primary osteoporosis.Results When stratified by sex, a negative correlation between obesity and primary osteoporosis was observed in males (χ2MH =26.845, P<0.001).Simultaneously, the prevalence of primary osteoporosis in subjects with II degree obese was higher than that in I degree obese (11.1% >6.6%).In females, no significant association between obesity and primary osteoporosis was observed (χ2MH =2.007, P=0.157).Without the stratification of gender, the data showed a negative correlation between obesity and primary osteoporosis (χ2MH =7.016,P=0.008).When stratified by age, each period ( except the age group over 70) showed that obesity and primary osteoporosis was negatively associated (χ2MH =4.152, 15.306, 10.212, 18.499, and 3.341, respectively; and P was 0.042, 0.001, 0.001, <0.001, and 0.068, respectively).Through the multivariate logistic regression analysis, after adjusting the gender and age, the results showed that obesity and primary osteoporosis was negatively associated (OR =0.703, 95%CI: 0.620 -0.799).Conclusion Osteoporosis is closely associated with obesity.Weight management is a public health problem in the prevention of osteoporosis, which should be paid more attention.
