目的:分析保留部分十二指肠在胰十二指肠切除术中的临床应用安全性与具体疗效。方法回顾性分析彭州市人民医院普通外科收治的34例在胰十二指肠手术中保留部分十二指肠患者的临床资料和影像学资料,总结该术式的优点与不足。结果全组无因手术死亡的病例,术后4例发生切口感染,2例发生切口疝,均行二期缝合治愈。随访3个月~5年,中位生存时间为25个月;包括:1年生存人数27例,生存率79.4%;3年生存人数7例,生存率20.6%;5年生存人数1例,生存率2.9%。结论保留部分十二指肠在胰十二指肠切除术极大的缩短了手术时间、改善患者预后,对年老体弱、重要脏器功能差的患者尤为适宜。

Objective To investigate the clinical results and safety of reservation of partial duodenum in pancreati-coduodenectomy. Methods The clinical data of 34 patients who were treated with pancreaticoduodenectomy with reservation of partial duodenum in this hospital were collected and retrospectively analyzed. The advantages and disadvantages of this tech-nique were summarized. Results No patients died in operation. There were 4 cases with wound infection and 2 cases with inci-sional hernia after surgery,which were consequently cured by secondary suture. Following up of 3 months to 5 years,the middle surviving time was 25 months. There were 27 cases(79. 4% )with 1-year censor ratio,7 cases(20. 6% )with 3-year censor ratio,and 1 cases(2. 9% )with 5-year censor ratio. Conclusion The reservation of partial duodenum can greatly shorten the operation time,improve the prognosis of the patients in pancreaticoduodenectomy,which is particularly suitable for the senile and frail patients.
