目的：探讨文拉法辛联合胰岛素泵治疗伴焦虑抑郁2型糖尿病的临床疗效及安全性。方法：将确诊为2型糖尿病患者伴焦虑抑郁患者59例随机分为两组，均采用胰岛素泵治疗2周，之后改为优泌乐25皮下注射，治疗组同时口服文拉法辛，观察8周；治疗前后分别作 SAS、SDS、皮质醇（ CRO）、白细胞介素-6（ L-6），超敏C-反应蛋白（ hsC-RP ）比较，观察血糖达标时间及胰岛素用量情况，观察治疗后两组不良反应。结果：治疗前两组SAS、SDS评分均明显高于国内常模（ P＜0．01），治疗后治疗组SAS、SDS、CRO、L-6、hsC-RP 较治疗前有统计学意义（P＜0．05），但对照组较治疗前无显著性差异（ P＞0．05）；治疗组血糖达标时间及胰岛素用量均明显少于对照组，差异有统计学意义（ P＜0．05）；两组不良反应发生率不高，均为轻度，能耐受。结论：文拉法辛联合胰岛素泵治疗伴焦虑、抑郁的2型糖尿病起效快，效果肯定，不良反应少，值得推广使用。
목적：탐토문랍법신연합이도소빙치료반초필억욱2형당뇨병적림상료효급안전성。방법：장학진위2형당뇨병환자반초필억욱환자59례수궤분위량조，균채용이도소빙치료2주，지후개위우비악25피하주사，치료조동시구복문랍법신，관찰8주；치료전후분별작 SAS、SDS、피질순（ CRO）、백세포개소-6（ L-6），초민C-반응단백（ hsC-RP ）비교，관찰혈당체표시간급이도소용량정황，관찰치료후량조불량반응。결과：치료전량조SAS、SDS평분균명현고우국내상모（ P＜0．01），치료후치료조SAS、SDS、CRO、L-6、hsC-RP 교치료전유통계학의의（P＜0．05），단대조조교치료전무현저성차이（ P＞0．05）；치료조혈당체표시간급이도소용량균명현소우대조조，차이유통계학의의（ P＜0．05）；량조불량반응발생솔불고，균위경도，능내수。결론：문랍법신연합이도소빙치료반초필、억욱적2형당뇨병기효쾌，효과긍정，불량반응소，치득추엄사용。
Objective:To dicuss the clinical effects and safety of venlafaxine combined insulin pump in treatment of type 2 diabetes with depression .Method:59 cases of type 2 diabetes with depression were di-vided into two groups .Both subjects were treated with insulin pump for two weeks , and then subcutaneously injected recombinant human insulin lispro injection 25R , treatment group simultaneously took venlafaxi-neied, studied for 8 weeks.Compared the indexs of SAS ,SDS,CRO,IL-6,hsC-RP pre and post treatment , studied the time of reaching target of glucose ,insulin dose and side effect between two groups .Result:SAS, SDS scores were both singnificantly higher than domestic level before treatment ( P<0.01) .Pre and post treat-ment SAS,SDS,CRO,IL-6,hsC-RP of treatment group had significant difference (P<0.05), but not in con-trol group(P>0.05).The time of reaching target of glucose and insulin dose of treatment group were lower than control group .The incidence of side effects were low and could tolerate .Conclusion:Venlafaxine com-bined insulin pump in the treatment of type 2 diabetes is effective and safety ,worth popularizing .
