目的：评价枢椎椎弓根拉力螺钉治疗II型Hangman骨折的临床疗效。方法对我科12例II型Hangman骨折采用双侧枢椎椎弓根拉力螺钉治疗，其中II型8例，IIa型4例，术前、术后及随访时分别评估脊髓神经功能及影像学资料。结果所有患者均获得随访，评均随访15个月（6~24月），手术平均时间65min（50~80min），无脊髓神经损伤及其他并发症，骨折均在术后3~6个月愈合，未出现螺钉断裂、松动，未见C2/3失稳及后凸畸形形成。结论枢椎椎弓根拉力螺钉能够安全、有效的治疗II型Hangman骨折。
목적：평개추추추궁근랍력라정치료II형Hangman골절적림상료효。방법대아과12례II형Hangman골절채용쌍측추추추궁근랍력라정치료，기중II형8례，IIa형4례，술전、술후급수방시분별평고척수신경공능급영상학자료。결과소유환자균획득수방，평균수방15개월（6~24월），수술평균시간65min（50~80min），무척수신경손상급기타병발증，골절균재술후3~6개월유합，미출현라정단렬、송동，미견C2/3실은급후철기형형성。결론추추추궁근랍력라정능구안전、유효적치료II형Hangman골절。
Objective To discuss the clinical ef ects and value of surgical treatment by direct transpedicular osteosynthesis with lag screw for II Hangman,s Fracture. Method:12 patients with with acute traumatic spondylolisthesis of axis were treated by direct transpedicular osteo -synthesis with lag screw .8 patients classified as type II,4 patients classified as type IIa. Its image data and the function of spinal cord and nerves were col ected at pre-op,post-op and fol ow-up respectively. Result:Al patients were fol owed up,mean fol ow-up time is 15 months,it cost an average time of 65 minutes (50-80min)to finish the procedure. al cases gained bony fusion 3-6 months later and didn't any postoperation complicat -ions. The range of neck rotation was stored normal with no c2/3 instability、screw loosing or broking、kyphosis found. Conclusion:Transpedicular lag screw fixation can treated II Hangman,s Fracture safely and ef ectively.
