目的：利用医院-社区-家庭一体化合作管理模式对糖尿病患者进行管理，并进行效果评价。方法将76例糖尿病患者进行分组管理，比较实验组和对照组之间差异。结果实验组患者干预前后空腹血糖下降有显著性意义(P<0.05)，对照组的空腹血糖、甘油三脂、总胆固醇水平均呈上升趋势，具有显著性(P<0.05)。结论一体化管理模式促进了糖尿病患者相关知识、态度、行为的正向改变，有效控制了糖尿病患者的病情。
목적：이용의원-사구-가정일체화합작관리모식대당뇨병환자진행관리，병진행효과평개。방법장76례당뇨병환자진행분조관리，비교실험조화대조조지간차이。결과실험조환자간예전후공복혈당하강유현저성의의(P<0.05)，대조조적공복혈당、감유삼지、총담고순수평균정상승추세，구유현저성(P<0.05)。결론일체화관리모식촉진료당뇨병환자상관지식、태도、행위적정향개변，유효공제료당뇨병환자적병정。
Objective:To use the hospital-Community-Family Integrated cooperative management model for diabetics to manage and conduct Evaluation. Methods:76 cases of patients with diabetes management division, comparison between the experimental group and control group dif erences. Results:The experimental group before and after intervention in patients with fasting blood glucose decreased significantly (P<0.05), control group, fasting blood glucose, triglycerides, total cholesterol levels showed an upward trend, with a significant (P<0.05). Conclusion:ntegrated management model to promote the diabetes-related knowledge, at itudes, behaviors positive change, ef ective control of the diabetic patient's condition.
