目的观察微创置管胸膜腔注入顺铂、香菇多糖联合凝血酶治疗恶性胸腔积液的近期疗效及安全性.方法将41例恶性胸腔积液患者完全随机分为试验组(21例)和对照组(20例),胸腔微创置管引流,排尽胸腔积液后,试验组胸腔内先后注入顺铂40 mg、香菇多糖4 mg24 h后注入凝血酶1000 U胸腔内注入;对照组仅注入相同剂量顺铂和香菇多糖.每周1次,4次后观察临床疗效、生活质量及不良反应.结果试验组治疗完全缓解率[57.1％(12/21)]、生活质量改善率[66.7％(14/21)]均高于对照组[25.0％(5/20)和35.0％ (7/20)] (P ＜0.05).结论微创置管引流胸腔积液后,胸膜腔内注入顺铂联合香菇多糖并凝血酶治疗恶性胸腔积液较顺铂联合香菇多糖完全缓解率更高,明显改善患者生活质量.

Objective To demonstrate the clinical curative effect and safety of minitraumatic tubeinstallation with intrapleural injection cisplatin and lentinan together with thrombin treating malignant pleural effusion.Methods Forty-one patients with malignant pleural effusion were randomly divided into experiment group and control group after thoracentesis.Intrapleural injection of cisplatin and lentinan with thrombin were administered in the experiment group; cisplatin and lentinan were given in the control group.The therapies were repeated every week for a total of four times.The clinical efficacy,quality of life and side-effects were observed.Results The complete remisson rate and improvement rate of life quality in experiment group were higher than those in control group[57.1％ (12/21)vs 25.0％ (5/20),66.7％(14/21)vs 35.0％ (7/20),P＜0.05].Conclusions By minitraumatic tube installation and drainage,the use of cisplatin and lentinan in combination with thrombin can achieve better results,including quality of life,than administration of cisplatin and lentinan in patients with malignant pleural effusion without increasing danger or adverse effects.
