目的：探讨骨水泥型长柄双极人工股骨头置换治疗高龄股骨粗隆间骨折的临床疗效。方法：回顾性分析2009年6月-2012年6月采用人工股骨头置换治疗的36例股骨粗隆间骨折患者的资料，其中男15例，女21例，年龄81~95岁，平均85.7岁。骨折按Evan’s分类：Ⅲa型11例，Ⅲb型16例，Ⅳ型5例，V型4例。结果：本组病例手术均较顺利，住院期间无死亡病例。术后随访6~18个月，平均13.5个月，无假体松动、关节脱位，无钢丝松动及断裂。髋关节功能恢复良好，局部疼痛缓解或消失，生活能基本自理，优良率为86.1%。结论：骨水泥型长柄双极人工股骨头置换治疗高龄股骨粗隆间骨折，卧床时间短，可早期离床负重活动，减少并发症，提高患者术后生活质量，但应严格掌握适应证。
목적：탐토골수니형장병쌍겁인공고골두치환치료고령고골조륭간골절적림상료효。방법：회고성분석2009년6월-2012년6월채용인공고골두치환치료적36례고골조륭간골절환자적자료，기중남15례，녀21례，년령81~95세，평균85.7세。골절안Evan’s분류：Ⅲa형11례，Ⅲb형16례，Ⅳ형5례，V형4례。결과：본조병례수술균교순리，주원기간무사망병례。술후수방6~18개월，평균13.5개월，무가체송동、관절탈위，무강사송동급단렬。관관절공능회복량호，국부동통완해혹소실，생활능기본자리，우량솔위86.1%。결론：골수니형장병쌍겁인공고골두치환치료고령고골조륭간골절，와상시간단，가조기리상부중활동，감소병발증，제고환자술후생활질량，단응엄격장악괄응증。
Objective:To discuss the clinical effect of cemented long stem bipolar artificial femoral head replacement for the treatment of intertrochanteric fractures.Method:A retrospective analysis from June 2009 to June 2012 by artificial femoral head replacement for the treatment of 36 cases of intertrochanteric fracture,male 15 cases,female 21 cases,age 81-95 years,average 85.7 years old.Fracture classification according to Evan’s:11 cases of TypeⅢa,16 cases of typeⅢb,5 cases of type IV,4 cases of type V.Result:The patients had successful operation,no patients died during hospitalization.Patients were followed up for 6-18 months,average 13.5 months,no prosthesis loosening,dislocation,no wire looseness and breakage.The hip joint function recovered well,local pain relieved or disappeared,the life can take care of themselves,the excellent and good rate was 86.1%.Conclusion:Cemented long stem bipolar artificial femoral head replacement for treatment of femoral intertrochanteric fractures in the elderly,bedridden time is short,can get out of bed early weight-bearing activities, reduce the complications,improve the quality of life of patients,but the indication should be strictly controlled.
