目的:探讨乌司他丁(UTI)对盲肠结扎穿孔术(CLP)所致大鼠脓毒症急性肾损伤(AKI)的保护作用。方法SD健康雄性大鼠55只,按随机化原则分成3组:正常对照组5只、模型组25只、乌司他丁治疗组25只,后两组再随机分为5个时间点(1h、6h、12h、24h、48h),每组每时间点各5只,采用盲肠结扎穿孔术复制脓毒症模型,乌司他丁治疗组,造模后立即给予乌司他丁10万U/kg,尾静脉注射,分别在各个时间点采血、留尿标本,进行肾功能血肌酐(Scr)、尿素(Urea),尿肾损伤分子(KIM-1)、白细胞介素-18(IL-18)和中性粒细胞明胶酶相关脂质运载蛋白(NGAL)检测分析。结果与模型组相比,乌司他丁治疗组大鼠各时间点Scr、Urea,尿KIM-1、IL-18、NGAL浓度均显著降低,差异均有统计学意义(P<0.05)。结论乌司他丁对脓毒症所致急性肾损伤具有一定的肾脏保护作用。

목적:탐토오사타정(UTI)대맹장결찰천공술(CLP)소치대서농독증급성신손상(AKI)적보호작용。방법SD건강웅성대서55지,안수궤화원칙분성3조:정상대조조5지、모형조25지、오사타정치료조25지,후량조재수궤분위5개시간점(1h、6h、12h、24h、48h),매조매시간점각5지,채용맹장결찰천공술복제농독증모형,오사타정치료조,조모후립즉급여오사타정10만U/kg,미정맥주사,분별재각개시간점채혈、류뇨표본,진행신공능혈기항(Scr)、뇨소(Urea),뇨신손상분자(KIM-1)、백세포개소-18(IL-18)화중성립세포명효매상관지질운재단백(NGAL)검측분석。결과여모형조상비,오사타정치료조대서각시간점Scr、Urea,뇨KIM-1、IL-18、NGAL농도균현저강저,차이균유통계학의의(P<0.05)。결론오사타정대농독증소치급성신손상구유일정적신장보호작용。
Objective To investigate the effects of ulinastatin on acute kidney injury in rats with sepsis. Methods Fifty-five healthy male sprague-dawley rats were randomly divided into 3 groups: normal control, model group, ulinastatin group.The rat models of sepsis were established by caecal ligation and puncture(CLP).In treatment groups, rats were treated with ulinastatin through caudalis injection after CLP surgery.Each group was divided into 1, 6, 12, 24, 48 hour subgroups with 5 rats. Rats were sacrificed and tissue samples including kidney and blood samples were collected.The kidney function,urine level of kidney injury molecule-1(KIM-1), neutrophil gelatinase lipocalin(NGAL) and interleukin-18(IL-18) were measured.Results Compared with CLP model group, renal function, the levels of KIM-1, NGAL and IL-18 in urine, significantly decresed in Ulinasta group(P < 0.05). Conclusion Ulinasta can improving renal function duringAKI in the rat model of sepsis.
