目的:探讨参与过敏性紫癜( HSP )患儿肾脏损害的可能危险因素,为预防过敏性紫癜肾脏损害提供理论依据。方法60例过敏性紫癜患儿入院后检查尿常规、血尿素氮( BUN )、肌酐( Scr )、C 反应蛋白( CRP )、血沉( ESR )、谷草转氨酶(AST)、磷酸肌酸同工酶(CK-MB)、血胱抑素C(CysC)、尿IV型胶原纤维蛋白(尿IV-C)、尿N-乙酰-β-D-氨基葡萄糖苷酶(尿NAG酶)等多项生化指标,将所有资料进行线性相关及多元线性回归分析。结果60例过敏性紫癜患儿肾脏损害的检出率为35%,线性相关分析显示:总蛋白( TP)、总胆红素( TBIL)、直接胆红素( DBIL)、间接胆红素( IBIL)、谷丙转氨酶( ALT)、Scr与BUN呈正相关(r值分别为0.23,0.16,0.18,0.22,0.12,0.14,均P<0.05),且白蛋白(ALB)、球蛋白(GLB)、CRP、ESR、AST、CK-MB亦与BUN呈正相关(r值分别为0.38,0.32,0.56,0.49,0.46,0.35,均P<0.01);进一步多元线性回归显示CRP、AST、CK-MB及ESR与BUN密切相关,其中决定系数R=0.901,R2=0.812。结论 CRP、AST、CK-MB及ESR可能与HSP患儿肾脏损害的程度相关联,在HSP的治疗中对以上指标应该引起足够重视。

목적:탐토삼여과민성자전( HSP )환인신장손해적가능위험인소,위예방과민성자전신장손해제공이론의거。방법60례과민성자전환인입원후검사뇨상규、혈뇨소담( BUN )、기항( Scr )、C 반응단백( CRP )、혈침( ESR )、곡초전안매(AST)、린산기산동공매(CK-MB)、혈광억소C(CysC)、뇨IV형효원섬유단백(뇨IV-C)、뇨N-을선-β-D-안기포도당감매(뇨NAG매)등다항생화지표,장소유자료진행선성상관급다원선성회귀분석。결과60례과민성자전환인신장손해적검출솔위35%,선성상관분석현시:총단백( TP)、총담홍소( TBIL)、직접담홍소( DBIL)、간접담홍소( IBIL)、곡병전안매( ALT)、Scr여BUN정정상관(r치분별위0.23,0.16,0.18,0.22,0.12,0.14,균P<0.05),차백단백(ALB)、구단백(GLB)、CRP、ESR、AST、CK-MB역여BUN정정상관(r치분별위0.38,0.32,0.56,0.49,0.46,0.35,균P<0.01);진일보다원선성회귀현시CRP、AST、CK-MB급ESR여BUN밀절상관,기중결정계수R=0.901,R2=0.812。결론 CRP、AST、CK-MB급ESR가능여HSP환인신장손해적정도상관련,재HSP적치료중대이상지표응해인기족구중시。
Objective To investigate the possible risk factors involving the renal damage of children with henoch -schonlein purpura ( HSP) and to provide theoretical basis for the prevention of renal damage .Methods Several biochemical indexes of 60 HSP cases , including routine urinalysis, BUN, Scr, CRP, ESR, AST, CK-MB, CysC, urinary IV-C and urinary NAG, were detected after hospitalization . Linear correlation analysis and multiple linear regression analysis were conducted on all of the data .Results The detection rate of renal damage among 60 cases of HSP was 35%.Linear correlation analysis showed that BUN was positively related with TP , TBIL, DBIL, IBIL, ALT and Scr (r value was 0.23, 0.16, 0.18, 0.22, 0.12 and 0.14, respectively, all P<0.05), and that RUN was positively related with ALB, GLB, CRP, ESR, AST and CK-MB (r value was 0.38, 0.32, 0.56, 0.49, 0.46 and 0.35, respectively, all P<0.01). Further multivariate linear regression analysis showed that CRP , AST, CK-MB and ESR were closely related with BUN , and the coefficient of determination R was 0.901 and R2 was 0.812.Conclusion CRP, AST, CK-MB and ESR may be related with the severity of renal damage of HSP children .In the treatment of HSP enough attention should be paid to the above factors .
